Ölümcül Ventriküler Aritmilerde Prekordiyal Yumruk Uygulamasının Etkinliği
Özet
Kardiyopulmoner resüsitasyon (KPR) ani kardiyak ölümde kurtarıcı tek tedavidir. Alternatif KPR teknikleri olarak öksürük KPR’si ve prekordiyal yumruk prosedürleri bildirilmiştir. Prekoridyal yumruk, kurtarıcının hastanın göğsüne yumruğunun tabanı ile vurduğu bir dış göğüs kompresyon şeklidir. Bu prosedürün amacı, kalp kasına şok sağlayarak kalbin normal ritmini geri kazandırmaktır. Kontrollü çalışmalar ve önemli kanıtlar olmamasına rağmen prekordiyal yumruk hala uygulanmaktadır ve resüsitatif tıpta faydası tartışmalı bir konu olmaya devam etmektedir. Prekordiyal yumruk tanık olunan, izlenen, kararsız VT olan ve hemen bir defibrilatör bulunmadığında hastalar için potansiyel olarak hayat kurtarıcı bir prosedürdür. Fakat uygulama alanı, endikasyonları ve faydaları son derece sınırlıdır. Prekordiyal yumruk KPR veya defibrilasyonu geciktirmemeli ya da bu müdahalelere alternatif olarak düşünülmemelidir.
