Doğum Eyleminde Ağrının Değerlendirilmesinde Kullanılan Ölçekler

Özet

Doğum eylemi ve bu süreçte yaşanan ağrı, kadınların hayatı boyunca deneyimleyebileceği en şiddetli ve karmaşık acılardan biridir. Doğum ağrısının doğru yönetilmesi ve obstetrik kararların sağlıklı verilebilmesi için ağrının objektif olarak değerlendirilmesi büyük önem taşımaktadır. Kadının kendi ifadesi en güvenilir gösterge olsa da ağrıyı tam tanımlayamadığı durumlarda geçerli, güvenilir ve anlaşılır ağrı ölçeklerine ihtiyaç duyulmaktadır. Bu çalışmada, literatür incelenerek doğum ağrısının değerlendirilmesinde kullanılan çeşitli ölçeklerin özellikleri ayrıntılı olarak sunulmuştur. İnceleme kapsamında; algılanan ağrıyı 100 mm’lik bir çizgi üzerinde ölçen ve altın standart kabul edilen Görsel Kıyaslama Ölçeği (VAS), 15 ifadeden oluşan McGill Kısa Soru Formu (SF-MPQ) ve ağrının kabul edilebilirliğini renkli bir şema ile ölçen Kabul Semptomu Değerlendirme Ölçeği (KSDÖ) ele alınmıştır. Ayrıca, duruma özel ilk çok boyutlu psikometrik araç olan Angle Doğum Ağrısı Ölçeği ile gebenin mahremiyetine saygı duyarak sadece ebenin kasılma anındaki gözlemine dayanan Doğum Sırasında Ağrı İfadesi Derecelendirme Ölçeği (ESVADOPA) incelenmiştir. Sonuç olarak, standart ölçüm araçlarının kullanımının gereksiz medikal analjeziyi azaltacağı ve anne memnuniyetini artıracağı vurgulanmıştır.

Childbirth and the associated pain are defined as one of the most severe and complex painful experiences a woman can encounter throughout her life, directly impacting her overall birth experience. To manage labor pain effectively and contribute positively to the obstetric decision-making process, evaluating this subjective pain accurately is crucial. Although the woman's self-report is the most reliable indicator, valid and reliable pain scales are required to transform subjective intensity into objective data when women cannot fully describe their pain. This study aims to examine and present the characteristics of various scales used in assessing labor pain through a comprehensive literature review. Within this scope, the Visual Analogue Scale (VAS), which measures perceived pain on a 100 mm line and is accepted as the gold standard, the Short-Form McGill Pain Questionnaire (SF-MPQ) consisting of 15 emotional and affective statements, and the Acceptance Symptom Assessment Scale (ASAS), which evaluates whether severe pain is acceptable using a color gradient bar, are comprehensively analyzed. Furthermore, the Angle Labor Pain Questionnaire, developed as the first situation-specific multidimensional psychometric tool evaluating high pain experiences, and the Rating Scale of Pain Expression During Childbirth (ESVADOPA), which relies solely on the midwife's observation during a contraction without interrupting the woman's privacy, are thoroughly detailed. In conclusion, the standardization of these measurement tools is emphasized to optimize care quality, reduce unnecessary medical analgesia, and increase maternal satisfaction.
