Palyatif Dönemdeki Çocuk ve Hemşirelik Bakımı
Özet
Palyatif bakım, yaşamı tehdit eden ciddi hastalıklarla karşı karşıya kalan hasta ve ailelerinin fiziksel, psikososyal ve ruhsal sorunlarını erken teşhis, doğru değerlendirme ve tedavi yöntemleriyle hafifleterek yaşam kalitesini artırmayı amaçlayan kişi ve aile merkezli bütüncül bir yaklaşımdır. Tarihsel süreçte orta çağdaki din temelli gönüllü hareketlerle başlayan bu hizmet, Cicely Saunders'ın 1967'de modern hospis anlayışını başlatmasıyla gelişim göstermiş; Türkiye'de ise özellikle 2008 yılından itibaren geliştirilen ulusal programlar, projeler ve resmi yönergelerle sağlık sistemine entegre edilmeye başlanmıştır. Pediatrik palyatif bakım (PPB) ise yetişkinlerden farklı olarak çocukların sürekli değişen fiziksel, hormonal, bilişsel ve duygusal gelişim süreçlerine ve özel ihtiyaçlarına dikkat edilmesini gerektirir. Dünya genelinde her yıl milyonlarca çocuğun bu hizmete gereksinimi bulunmasına rağmen, özellikle düşük ve orta gelirli ülkelerde erişim oranları son derece sınırlıdır. Süreçte çocukla en çok zaman geçiren PPB hemşireleri; başta ağrı olmak üzere dispne, bulantı, anksiyete ve korku gibi semptomların farmakolojik ya da farmakolojik olmayan tekniklerle kontrol altına alınmasında, hasta ve aile eğitiminde, multidisipliner koordinasyonda ve yas sürecinin yönetiminde kritik roller üstlenmektedir. Sonuç olarak, çocuklara özgü bakım modellerinin literatüre kazandırılması, kanıta dayalı çok merkezli araştırmaların artırılması ve hemşirelere yönelik sertifikalı eğitim programları ile farkındalığın yaygınlaştırılması, bu hizmetlerin kalitesini ve entegrasyonunu artırmak adına büyük önem arz etmektedir.
Palliative care is a person- and family-centered holistic approach that aims to improve the quality of life of patients and their families facing life-threatening serious illnesses by preventing and alleviating physical, psychosocial, and spiritual problems through early identification, correct assessment, and treatment. Starting historically with faith-based voluntary movements in the Middle Ages, this service progressed with Cicely Saunders initiating the modern hospice concept in 1967; in Turkey, it began to be integrated into the healthcare system particularly from 2008 onwards through national programs, projects, and official guidelines. Pediatric palliative care (PPC), unlike adult care, requires attention to children's continuously changing physical, hormonal, cognitive, and emotional developmental processes and unique needs. Although millions of children worldwide need this service every year, access rates are extremely limited, especially in low- and middle-income countries. PPC nurses, who spend the most time with the child, play critical roles in controlling symptoms such as pain, dyspnea, nausea, anxiety, and fear using pharmacological or non-pharmacological techniques, as well as in patient and family education, multidisciplinary coordination, and grief management. Consequently, introducing child-specific care models to the literature, increasing evidence-based multi-center studies, and expanding awareness through certified training programs for nurses are of great importance to enhance the quality and integration of these services.
