Özet

Dünya Sağlık Örgütü verilerine göre çocuk istismarı; 18 yaş altındaki çocukların sağlığını, gelişimini ve güven duygusunu olumsuz etkileyen fiziksel, duygusal, cinsel istismar ve ihmal gibi kötü muamelelerin tümünü kapsamaktadır. Bu durum, mağdurlar üzerinde kısa vadede ölüme kadar varan yaralanmalara, uzun vadede ise kişilik bozuklukları, kaygı bozukluğu ve intihar eğilimi gibi ciddi psikolojik rahatsızlıklara yol açarak küresel bir sosyal sorun teşkil etmektedir. Çocuk istismarı; ihmal, fiziksel, duygusal ve cinsel istismar olarak dört ana grupta sınıflandırılmakta, özellikle fiziksel istismarda şüphe faktörü, tanı koymada kritik bir rol oynamaktadır. Sarsılmış bebek sendromu ve yapay bozukluk olarak da bilinen Munchausen by Proxy Sendromu gibi ağır klinik tablolarla karşılaşılabilen bu süreçte; yumuşak doku zedelenmeleri, farklı iyileşme evrelerindeki çoklu kemik ve kosta kırıkları fiziksel istismarın en belirgin göstergeleri arasında yer almaktadır. İstismarın önlenmesi ve kronikleşmeden durdurulması amacıyla, hastalarla karşılaşan tüm hekimlerin semptomları iyi analiz etmesi, multidisipliner bir yaklaşım benimsemesi, bulguları detaylıca kaydedip yasal mercilere bildirim yapması hukuki ve insani bir zorunluluktur.

According to the World Health Organization, child abuse encompasses all forms of ill-treatment, including physical, emotional, and sexual abuse, as well as neglect, that adversely affect the health, development, and sense of trust of children under the age of 18. This situation constitutes a global social problem, causing severe psychological disorders such as personality disorders, anxiety disorders, and suicidal tendencies in the long term, alongside injuries that can lead to death in the short term. Child abuse is classified into four main groups as neglect, physical, emotional, and sexual abuse, and the element of suspicion plays a critical role in diagnosing physical abuse in particular. In this process, where severe clinical conditions such as shaken baby syndrome and Munchausen by Proxy Syndrome—also known as factitious disorder imposed on another—can be encountered, soft tissue injuries and multiple bone or rib fractures at different stages of healing are among the most prominent indicators of physical abuse. In order to prevent abuse and stop it before becoming chronic, it is a legal and humane obligation for all physicians encountering these patients to analyze symptoms thoroughly, adopt a multidisciplinary approach, carefully record findings, and report them to legal authorities.
