Acil Müdahale Gerektiren Kırıklar: Genç Hastada Femur Boyun, Talus ve Skafoid Kırıkları

Özet

Genç hastalarda görülen ve ortopedik travmada acil müdahale gerektiren femur boyun, talus ve skafoid kırıkları, bu kemiklerin dolaşım yapılarının zayıf olması sebebiyle avasküler nekroz ve kaynamama gibi ciddi riskler barındırır. Yüksek enerjili travmalarla meydana gelen femur boyun kırıkları periost tabakası içermediğinden, ostenekroz riskini azaltmak adına ilk 24 saat içinde anatomik redüksiyon ve internal fiksasyon ile tedavi edilmelidir. Genellikle yüksekten düşme veya motorlu araç kazalarıyla oluşan ve subtalar ile ayak bileği çıkıklarının eşlik edebildiği talus boyun kırıklarında da yumuşak doku bütünlüğünü korumak ve dolaşımı düzeltmek için acil anatomik müdahale gereklidir. Karpal kemikler arasında en sık kırılan skafoid ise el üzerine düşme sonucu yaralanır ve retrograd bir kanlanmaya sahiptir. Skafoid bel ve proksimal kutup kırıklarında yetersiz beslenme kaynaklı kaynama gecikmesi sık yaşandığından, instabilite durumlarında erken cerrahi tespit ile revaskülarizasyon sağlanmalıdır. Her üç kırık tipinde de doğru radyolojik değerlendirme ve zamanında yapılan cerrahi, hastaların uzun dönemli fonksiyonel başarısını doğrudan belirlemektedir.

Femoral neck, talus, and scaphoid fractures, which are observed in young patients and require urgent intervention in orthopedic trauma, carry serious risks of avascular necrosis and nonunion due to the poor circulatory structures of these bones. Femoral neck fractures, occurring with high-energy traumas, lack a periosteal layer and must be treated with anatomical reduction and internal fixation within the first 24 hours to reduce the risk of osteonecrosis. In talus neck fractures, which usually result from high-ranking falls or motor vehicle accidents and can be accompanied by subtalar and ankle dislocations, urgent anatomical intervention is required to preserve soft tissue integrity and restore circulation. The scaphoid, the most frequently fractured carpal bone, is injured by falling on an outstretched hand and possesses a retrograde blood supply. Since delayed union due to malnutrition is common in scaphoid waist and proximal pole fractures, early surgical fixation should be performed in unstable cases to ensure revascularization. In all three fracture types, accurate radiological evaluation and timely surgery directly determine the long-term functional success of patients.
