Ortopedide Kırık ve Çıkıklar Neden Acildir?
Özet
Kırık ve çıkıklar, kemik bütünlüğünün ve çevre yumuşak dokuların zarar gördüğü, hızlı müdahale gerektiren hayati ortopedik acillerdir. Kırıklarda erken dönemde alçı, atel veya eksternal fiksatörler ile stabilizasyonun sağlanması; hücresel düzeyde kaynamama riskini azaltır, anatomik aksı koruyarak ödem, ağrı ve doku nekrozunu engeller. Politravma hastalarında özellikle pelvis ve femur gibi uzun kemik kırıkları, ciddi iç kanamalara ve hipovolemik şoka yol açarak ölümcül sonuçlar doğurabileceğinden, acil fiksasyon hayat kurtarıcıdır. Ayrıca erken müdahale, pulmoner emboli, derin ven trombozu ve yağ embolisi gibi uzamış immobilizasyona bağlı gelişebilecek mortalitesi yüksek sistemik komplikasyon risklerini anlamlı düzeyde düşürür. Eklem çıkıkları ise sinoviyal sıvı fonksiyonunu ve kıkırdak metabolizmasını hızla bozduğu için ilk saatlerde sedasyon altında kapalı redüksiyon ile tedavi edilmelidir. Zamanında müdahale edilmeyen çıkıklarda kas spazmı ve ödem nedeniyle açık cerrahi kaçınılmaz hale gelebilir. Hem kırık hem çıkıklarda, müdahale öncesi ve sonrasında nörovasküler muayenelerin tekrarlanarak kaydedilmesi, kalıcı sinir hasarı ve ekstremite kayıplarının önlenmesi açısından kritik bir öneme sahiptir.
Fractures and dislocations are vital orthopedic emergencies causing damage to bone integrity and surrounding soft tissues, thereby requiring prompt intervention. In fractures, ensuring stabilization early on with casts, splints, or external fixators reduces the risk of nonunion at the cellular level and prevents edema, pain, and tissue necrosis by preserving the anatomical axis. In polytrauma patients, fractures in long bones such as the pelvis and femur can lead to severe internal bleeding and hypovolemic shock, making emergency fixation lifesaving. Furthermore, early intervention significantly decreases the risk of systemic complications with high mortality, such as pulmonary embolism, deep vein thrombosis, and fat embolism associated with prolonged immobilization. Joint dislocations, which rapidly impair synovial fluid function and cartilage metabolism, must be treated with closed reduction under sedation within the first hours. In dislocations not intervened timely, open surgery may become inevitable due to muscle spasms and edema. In both fractures and dislocations, repeating and recording neurovascular examinations before and after the intervention is critical to preventing permanent nerve damage and limb loss.
