Özet

Alçı, atel uygulamaları ve iskelet traksiyonu gibi temel ortopedik immobilizasyon tekniklerini ele alan bu tıbbi metin, kırık ve çıkıkların tedavisinde yumuşak doku hasarını önlemeyi ve stabilizasyon sağlamayı amaçlayan kapalı redüksiyon ilkeleriyle başlamaktadır. Geçici tespit sağlayan ve akut inflamatuar ödem oluşumuna izin veren atel uygulamaları ile çok daha güçlü bir stabilizasyon sunarak nihai tedavide tercih edilen sirküler alçı teknikleri, kullanılan kalsiyum sülfat bazlı boz alçılar ve modern fiberglas materyaller üzerinden avantaj ve dezavantajlarıyla detaylandırılmaktadır. Doğru uygulama prosedüründe cilt koruması için alçı çorabı ve pamuk kullanımı, ekstremite boyutuna göre uygun kat sayılarının belirlenmesi ve avuç içi ile şekillendirme yapılması vurgulanırken, hatalı uygulamaların kompartman sendromu gibi iskemiye yol açabilen ciddi komplikasyonlara neden olabileceğine dikkat çekilmektedir. Metinde ayrıca üst ekstremitede ulnar/radial oluk, başparmak destekli, volar/dorsal ön kol, kısa/uzun kol alçı-atelleri ile buddy taping gibi spesifik uygulamalar, alt ekstremitede ise kısa/uzun bacak posterior atel ve sirküler alçı yöntemleri endikasyon ve pozisyonlarıyla açıklanmaktadır. Son bölümde ise pelvis ve alt ekstremite kırıklarında kas kuvvetini dengelemek, ödem ve ağrıyı kontrol etmek amacıyla kullanılan invazif bir yöntem olan iskelet traksiyonunun temel prensipleri, olekranon, distal femur, proksimal tibia ve kalkaneus gibi güvenli anatomik giriş bölgeleri ve işlem sonrası hasta bakımı prosedürleriyle sunulmaktadır.

This medical text, which covers fundamental orthopedic immobilization techniques such as casting, splinting, and skeletal traction, begins with the principles of closed reduction aimed at preventing soft tissue damage and providing stabilization in fractures and dislocations. Splint applications, which provide temporary fixation and accommodate acute inflammatory edema, and circular casting techniques, which offer stronger stabilization and are preferred for definitive treatment, are detailed with their advantages and disadvantages regarding calcium sulfate-based plaster of Paris and modern fiberglass materials. While the use of stockinettes and cotton padding for skin protection, determining appropriate layer counts based on extremity size, and molding with the palm are emphasized in the correct application procedure, it is highlighted that improper applications can cause serious complications like compartment syndrome that lead to ischemia. Furthermore, specific upper extremity applications like ulnar/radial gutter, thumb spica, volar/dorsal forearm, short/long arm casts-splints, and buddy taping, as well as lower extremity methods like short/long leg posterior splints and circular casts, are described with their indications and positions. In the final section, the basic principles of skeletal traction, an invasive method used to counteract muscle forces and control edema and pain in pelvis and lower extremity fractures, are presented along with safe anatomical entry zones like the olecranon, distal femur, proximal tibia, and calcaneus, as well as post-procedure patient care protocols.
