Culotte Stentleme Tekniği
Özet
Culotte stentleme tekniği, gerçek koroner bifurkasyon lezyonlarının (Medina 0,1,1 ve 1,1,1) tedavisinde karina ve yan dal ostiumunun ilaç salınımlı stent yapısıyla mükemmel şekilde kaplanmasını ve homojen ilaç dağılımını sağlayan, anjiyografik başarısı yüksek bir çift stentleme stratejisidir. Klasik yöntemde ana dal stentlemesi sonrası yan dal kaybına karşı geliştirilen bu teknik, günümüz klinik pratiğinde ilk stentin daha açılı olan yan dala yerleştirildiği "inverted culotte" yaklaşımıyla rutin hale gelmiştir. Prosedür sırasıyla; her iki dalın kılavuz tellerle geçilmesi, ilk stentin yerleştirilmesi, distal strattan zahmetli bir yeniden telleme (rewire) işlemi, ikinci stentin proksimal örtüşme bırakılarak yerleştirilmesi, proksimal optimizasyon tekniği (POT) ve kissing balon (KB) dilatasyonu adımlarını kapsar. Çift katmanlı metal yükünü ve buna bağlı stent trombozu riskini azaltmak amacıyla, yan dal stentinin ana dala sadece 2-3 mm taşacak şekilde konumlandırıldığı "mini culotte" tekniği de literatürde önemli bir modifikasyon olarak yer alır. Tekniğin başlıca zorlukları arasında stent gözeneklerinden yeniden geçişin zaman alıcı olması, belirgin çap uyumsuzluklarında veya kapalı hücreli tasarımlarda gelişebilen dairesel yetersiz genişleme (napkin ring formasyonu), kılavuz tel sıkışması/kırılması riskleri ve balon ilerletme güçlükleri bulunur. Başarı için açık hücreli stentlerin tercih edilmesi, POT balonunun neokarinaya tam hizalanması ve hapsedilen tellerin post-dilatasyon öncesi çıkarılması hayati önem taşır.
The culotte stenting technique is a highly effective double-stenting strategy used in true coronary bifurcation lesions (Medina 0,1,1 and 1,1,1) to ensure comprehensive coverage of the carina and side branch ostium while providing homogeneous drug delivery with drug-eluting stents. Originally designed as a bail-out option after main branch stenting, contemporary clinical practice routinely utilizes the "inverted culotte" approach, where the first stent is deployed into the more angulated side branch. The step-by-step procedural workflow includes wiring and modifying both branches, deploying the initial stent, performing a challenging rewiring through the distal struts, implanting the second stent with a proximal overlap, and executing the proximal optimization technique (POT) alongside kissing balloon (KB) dilatation. To mitigate the extensive double-layer metal burden and the associated risk of stent thrombosis, the "mini culotte" technique serves as a crucial modification, limiting the side branch stent protrusion into the main branch to merely 2-3 mm. Major technical challenges encompass time-consuming rewiring, "napkin ring formation" caused by severe diameter mismatches or closed-cell designs, jailed wire entrapment or fracture, and difficulties in tracking balloons or stents. Achieving procedural success strictly requires selecting open-cell stent platforms, precise orthogonal POT balloon alignment relative to the neocarina, and removing jailed wires prior to high-pressure post-dilatations to prevent fatal complications.
