Jinekolojik Kanserlerde Yaşam Sonu Dönem ve Hemşirelik Yaklaşımı: Palyatif ve Hospice Bakımı

Özet

Bu bölüm, jinekolojik malignitelerde yaşam sonu dönemde palyatif ve hospice bakımın gerekliliğini ele almaktadır. Onkolojik tedavilerdeki gelişmeler sağkalımı uzatmasına rağmen, ileri evre hastalarda semptom yükü ve yaşam kalitesinde kayıplar sürmektedir. Palyatif bakım, küratif tedavilerin ötesine geçerek ağrı ve diğer semptomların kontrolünü, psikososyal desteği ve spiritüel gereksinimlerin karşılanmasını amaçlayan bütüncül bir yaklaşımdır. Dünya Sağlık Örgütü ve Amerikan Klinik Onkoloji Derneği, bu bakımın erken entegrasyonunu önermektedir. Jinekolojik kanserli hastalarda ağrı, yorgunluk, bulantı, nefes darlığı gibi fiziksel semptomların yanı sıra kaygı, depresyon ve belirsizlik sıkça görülmektedir. Literatür, palyatif bakımın hasta memnuniyetini artırdığını, agresif girişimleri azalttığını ve yaşamın son döneminde onurlu bir süreç sağladığını göstermektedir. Hemşireler bu süreçte semptom yönetimi, eğitim, danışmanlık, savunuculuk ve manevi destek rolleriyle kritik rol üstlenmektedir.
