Özet

Merkel hücreli karsinom (MHK), epitelyal ve nöroendokrin özellikler barındıran, oldukça agresif seyirli ve nadir görülen bir cilt kanseri türüdür. Genellikle ileri yaştaki bireylerde, güneşe maruz kalan baş ve boyun bölgelerinde hızla büyüyen, ağrısız ve morumsu nodüller şeklinde ortaya çıkar. Hastalığın etiyolojisinde kronik ultraviyole (UV) ışınları ve Merkel hücreli polioma virüsü (MHPyV) temel onkojenik faktörler olarak tanımlanmıştır. Histopatolojik olarak küçük mavi yuvarlak hücreli tümör morfolojisi sergileyen MHK’nin kesin tanısında, perinükleer nokta boyanması gösteren CK20 başta olmak üzere immünohistokimyasal belirteçler kritik rol oynar. Yüksek lokal nüks ve uzak metastaz riski taşıyan bu hastalığın evrelemesi AJCC 8. edisyonuna göre klinik ve patolojik olarak yapılır. Erken evre lokal hastalıklarda temiz cerrahi marjinli eksizyon ve adjuvan radyoterapi öncelikli tedavi seçeneğiyken, sentinel lenf nodu biyopsisi nodal tutulumun tespiti için önerilir. Prognozu oldukça kötü olan metastatik evrede ise geleneksel kemoterapilerin yerini, sağkalım oranlarında belirgin artış sağlayan ve tedavi kılavuzlarında ilk sıraya yerleşen avelumab ve pembrolizumab gibi immün kontrol noktası inhibitörleri almıştır.

Merkel cell carcinoma (MCC) is a rare, highly aggressive cutaneous malignancy exhibiting epithelial and neuroendocrine features, primarily localizing in the sun-exposed head and neck areas of elderly individuals as rapidly growing, painless, purplish nodules. Chronic ultraviolet (UV) radiation and Merkel cell polyomavirus (MCPyV) constitute the primary etiological drivers in its oncogenesis. Histopathologically characterized as a small blue round cell tumor, its definitive diagnosis relies on immunohistochemical panels, with CK20 serving as the most specific marker showcasing a distinct perinucleolar dot-like staining pattern. Staged clinically and pathologically via the AJCC 8th edition, MCC carries a high risk for locoregional recurrence and early distant metastasis. While wide local excision combined with adjuvant radiotherapy remains the cornerstone for localized disease, sentinel lymph node biopsy is recommended for regional nodal evaluation. In the metastatic setting, where the prognosis is historically poor, immune checkpoint inhibitors such as avelumab and pembrolizumab have revolutionized management, demonstrating significant survival benefits over conventional chemotherapy and becoming the preferred first-line recommendations in modern guidelines.
