Özet

Mukozal malign melanom, melanositlerden köken alan, agresif seyirli ve nadir görülen bir dış kutanöz kanser türüdür. Tüm melanom vakalarının yalnızca küçük bir yüzdesini oluşturan bu tümörler, ultraviyole ışınlarından bağımsız olarak solunum, gastrointestinal ve genitoüriner sistemlerin mukozal epitellerinde gelişir. En sık baş ve boyun bölgesinde (özellikle nazal kavite ve paranazal sinüslerde) saptanırken, anorektal ve vulvovajinal alanlarda da ortaya çıkabilmektedir. Genellikle 50-80 yaş aralığındaki ileri yaş gruplarında tanı alan bu hastalığın etiyolojisi tam olarak bilinmemekle birlikte, kutanöz melanomların aksine düşük BRAF (%<10) ve belirgin şekilde yüksek KIT mutasyonu (%25) oranları ile genetik olarak ayrışır. Erken evre ve lokalize hastalıklarda temel küratif tedavi yaklaşımı, negatif cerrahi sınırların hedeflendiği geniş lokal cerrahi eksizyondur. Ancak, tümörün anatomik yerleşim kısıtlılıkları nedeniyle tam rezeksiyon çoğunlukla zordur ve adjuvan radyoterapinin lokal kontrolü artırmasına rağmen genel sağkalıma katkısı belirsizdir. Metastatik veya rezeke edilemeyen evrelerde ise prognoz oldukça kötüdür; tedavide nivolumab ve ipilimumab kombinasyonu gibi immünoterapiler ilk basamakta tercih edilmekte, mutasyon durumuna göre imatinib gibi hedefe yönelik ajanlar veya kemoterapi rejimleri devreye girmektedir.

Mucosal malignant melanoma is an aggressive, rare subtype of non-cutaneous cancer arising from melanocytes located in mucosal surfaces. Accounting for only a small percentage of all melanoma diagnoses, these tumors develop independently of ultraviolet radiation within the mucosal epithelium of the respiratory, gastrointestinal, and genitourinary tracts. While most commonly detected in the head and neck region—specifically within the nasal cavity and paranasal sinuses—they also manifest in anorectal and vulvovajinal regions. Typically diagnosed in elderly populations between 50 and 80 years of age, the exact etiology remains unknown. Distinct from cutaneous melanomas, they exhibit low BRAF mutation rates (<10%) and significantly higher rates of KIT alterations (~25%). For localized disease, the primary curative approach is wide surgical excision aiming for negative margins. However, achieving complete resection is frequently complicated by anatomical constraints, and while adjuvant radiotherapy improves local control, its impact on overall survival remains uncertain. Metastatic or unresectable mucosal melanoma carries a poor prognosis; frontline management prioritizes combination immunotherapies like nivolumab and ipilimumab, while subsequent options include targeted agents like imatinib for KIT-mutated cases or systemic chemotherapy.
