İnce Bağırsak Tümörleri
Özet
İnce bağırsaklar, gastrointestinal sistemde en geniş yüzey alanına sahip olmasına rağmen, tüm gastrointestinal tümörlerin yalnızca %1-2'sini oluşturacak şekilde nadiren neoplazm geliştirirler. Özgün belirtiler göstermemeleri nedeniyle genellikle geç tanı alan bu tümörler; malign (adenokarsinom, nöroendokrin tümörler, lenfomalar, sarkomlar) ve benign (adenom, lipom vb.) olarak iki ana gruba ayrılır. Epidemiyolojik olarak son 40 yılda görülme sıklığı ikiye katlanmış, özellikle nöroendokrin tümörlerin (NET) insidansında belirgin bir artış yaşanarak adenokarsinomlar geride bırakılmıştır. Histolojik alt tipler incelendiğinde; adenokarsinomlar daha çok duodenumda yerleşirken alkol, sigara ve diyet faktörleriyle ilişkilidir. En sık ileumda görülen iyi diferansiye NET'ler ise karsinoid sendroma yol açabilen ve somatostatin analogları ile tedavi edilen heterojen bir gruptur. Sarkomların %85'inden fazlasını imatinib tedavisine yanıt veren gastrointestinal stromal tümörler (GİST) oluştururken, primer lenfomalar B hücreli histolojide olup ileumda yoğunlaşır. Tanıda BT/MR enteroklizis, kapsül endoskopisi ve PET-BT gibi modern yöntemler öne çıkarken; metastatik olmayan vakalarda temel cerrahi yaklaşım geniş R0 rezeksiyonu ve lenf nodu diseksiyonudur.
Despite having the largest surface area in the gastrointestinal system, the small intestine rarely develops neoplasms, accounting for only 1% to 2% of all gastrointestinal tumors. These tumors are usually diagnosed at an advanced stage due to non-specific symptoms and are classified as malignant (adenocarcinoma, neuroendocrine tumors, lymphomas, sarcomas) and benign (adenoma, lipoma, etc.). Epidemiologically, the incidence has doubled over the last 40 years, with neuroendocrine tumors (NETs) significantly increasing and surpassing adenocarcinomas. Regarding histological subtypes, adenocarcinomas are predominantly located in the duodenum and associated with alcohol, smoking, and dietary habits. NETs, most commonly found in the ileum, represent a heterogeneous group that can cause carcinoid syndrome and are primarily treated with somatostatin analogs. Sarcomas comprise mostly gastrointestinal stromal tumors (GISTs), which respond well to imatinib therapy, whereas primary lymphomas are predominantly B-cell type and located in the ileum. Modern diagnostic tools include CT/MR enteroclysis, capsule endoscopy, and PET-CT, while wide R0 surgical resection with lymph node dissection remains the primary treatment option for non-metastatic cases.
