Özet

Medulloblastom, çocukluk çağının en sık karşılaşılan malign beyin tümörü olup santral sinir sisteminin agresif ve yüksek dereceli (Evre 4) embriyonel tümörleri arasında sınıflandırılmaktadır. Genellikle serebellum veya dördüncü ventrikül çevresinde lokalize olan bu tümör, anatomik yerleşimi nedeniyle obstrüktif hidrosefaliye yol açarak baş ağrısı, bulantı ve ataksi gibi intrakranial basınç artışı semptomlarıyla klinik olarak prezente olur. Teşhis ve evreleme süreçlerinde detaylı nörolojik muayenenin yanı sıra kontrastlı manyetik rezonans görüntüleme (MRG) ve beyin omurilik sıvısı (BOS) sitopatolojisi hayati önem taşımaktadır. Histopatolojik olarak klasik, desmoplastik/nodüler, yaygın nodülariteli ve büyük hücreli/anaplastik varyantlara ayrılan hastalık; günümüzde gen ekspresyon profillerine göre WNT-aktif, SHH-aktif, Grup 3 ve Grup 4 olmak üzere dört ana moleküler alt grupta incelenir. Bu moleküler sınıflama, hastaların risk stratifikasyonunu belirlemede en belirleyici faktör olup WNT grubu mükemmel bir prognoza işaret ederken, nüks ve metastaz eğilimi yüksek olan Grup 3 en kötü prognoza sahiptir. Tedavi yönetiminde temel strateji, maksimal güvenli cerrahi rezeksiyonu takiben kraniyospinal radyoterapi ve sistemik kemoterapiden oluşan kombine modalitelerin uygulanmasıdır. Güncel teknolojik gelişmeler ve risk gruplarına göre optimize edilen kemoradyoterapi rejimleri sayesinde uzun süreli sağkalım oranlarında kayda değer başarılar elde edilse de, tedavi sonrasında gelişebilecek geç dönem nörotoksik komplikasyonların yönetimi kritik bir önem arz etmektedir.

Medulloblastoma is the most prevalent malignant brain tumor in children, categorized as an aggressive, high-grade (Grade 4) embryonal tumor of the central nervous system. Primarily localized within the cerebellum or the fourth ventricle, its proximity frequently causes obstructive hydrocephalus, presenting clinically with symptoms of increased intracranial pressure such as headache, vomiting, and ataxia. Diagnostic and staging protocols heavily rely on comprehensive neurological examinations, contrast-enhanced magnetic resonance imaging (MRI), and cerebrospinal fluid (CSF) cytopathology to detect leptomeningeal dissemination. Histopathologically differentiated into classic, desmoplastic/nodular, extensively nodular, and large cell/anaplastic variants, contemporary medicine classifies the disease into four major molecular subgroups based on gene expression: WNT-activated, SHH-activated, Group 3, and Group 4. This molecular stratification serves as a pivotal prognostic determinant, where the WNT subgroup demonstrates an excellent survival outcome exceeding ninety percent, whereas Group 3 exhibits a highly aggressive metastatic nature and the poorest prognosis. The cornerstone of therapeutic management involves a multidisciplinary approach encompassing maximal safe surgical resection followed by craniospinal irradiation and adjuvant systemic chemotherapy. Although advanced surgical techniques and risk-adapted chemoradiotherapy regimens have remarkably improved long-term survival rates to approximately seventy-five percent, mitigating late-term toxic complications and preserving the quality of life remain critical challenges in patient survivorship.
