Özet

Zirkonya abutmentler, doğal diş görünümü, yüksek mekanik direnci, biyouyumluluğu ve düşük plak akümülasyonu sayesinde implant destekli dental restorasyonlarda titanyuma estetik bir alternatif sunar. Özellikle bukkal yumuşak doku kalınlığının 2 mm’den az olduğu vakalarda ve yüksek gülme hattına sahip hastalarda gri yansımayı önleyerek estetik başarıyı artırır. Yapısal olarak tek parçalı veya titanyum tabanlı (hibrit) iki parçalı tasarımlarla üretilebilen bu dayanaklar, CAD/CAM teknolojisiyle kişiye özel hazırlanarak marjinal dokulara optimum destek ve daha hassas uyum sağlar. Klinik çalışmalar internal ve konik bağlantılı sistemlerin kuvvet dağılımında daha kararlı olduğunu, mekanik yüklemelere karşı direnç için duvar kalınlığının 0.5 mm'nin altına düşmemesi gerektiğini göstermektedir. Materyalin inert yapısı nedeniyle simantasyon öncesi alümina parçacıklarıyla kumlama ve fosfat monomerli (MDP) adezivlerin kullanımı klinik bağlantı başarısını optimize eder. Uzun dönemli takiplerde zirkonyanın yumuşak doku iyileşmesini hızlandırdığı, peri-implant sağlığını koruduğu ve krestal kemik kaybını titanyuma kıyasla önemli ölçüde azalttığı saptanmıştır.

Zirconia abutments offer an aesthetic alternative to titanium in implant-supported dental restorations due to their natural tooth appearance, high mechanical resistance, biocompatibility, and low plaque accumulation. Especially in cases where buccal soft tissue thickness is less than 2 mm and in patients with a high smile line, they enhance aesthetic success by preventing gray discoloration. Manufactured as one-piece or titanium-based (hybrid) two-piece designs, these abutments can be customized via CAD/CAM technology to provide optimum support to marginal tissues and a more precise fit. Clinical studies indicate that internal and conical connection systems are more stable in load distribution, and the wall thickness must not fall below 0.5 mm to resist mechanical loads. Due to the inert nature of the material, alumina particle airborne-particle abrasion and the use of phosphate monomer-containing (MDP) adhesives prior to cementation optimize clinical bonding success. Long-term follow-ups have revealed that zirconia accelerates soft tissue healing, preserves peri-implant health, and significantly reduces crestal bone loss compared to titanium.
