Dental İmplantlarda Osseopersepsiyon

Çiğneme, stomatognatik sistemin temel fonksiyonlarından biri olup doğal dişlerin kaybıyla bozulabilmekte, bu durum protetik diş hekimliği ile telafi edilmektedir. Günümüzde konvansiyonel tam protezlerin yerine implant destekli protezler, hastaların çiğneme fonksiyonunu ve yaşam kalitesini artırmak için sıklıkla tercih edilen güvenilir bir alternatif haline gelmiştir. Diş çekimi sonrasında periodonsiyumdaki mekanoreseptörlerin kaybıyla periferal geribildirim yolları ciddi zarar görse de, implant uygulanan hastalarda zamanla "osseopersepsiyon" adı verilen özel bir duyusal algılama gelişmektedir. Osseopersepsiyon, implant çevresindeki kemik, periost, kas ve eklemlerde bulunan sinir sonlanmalarının aktivasyonu sayesinde kemik destekli protezlerle iletilen harici uyaranların bilinçli olarak algılanmasıdır. Histolojik ve nörofizyolojik çalışmalar, yükleme sonrasında implant çevresindeki kemik dokusunda yeni sinir liflerinin rejenere olabildiğini ve kuvvetin kemiğe direkt iletilmesiyle oluşan mikro deformasyonların reseptörleri uyardığını göstermektedir. İmplantların aktif dokunma eşik değerleri doğal dişlerden yüksek olsa da, implant desteği tam protezlere kıyasla taktil hassasiyeti ve ısırma kuvvetini yaklaşık üç kat artırarak doğal dişlerin değerlerine yaklaştırır ve nöromüsküler adaptasyon sağlar. Sonuç olarak, peri-implant innervasyon ve merkezi sinir sisteminin plastisitesi sayesinde duyu-motor fonksiyonlar kısmen restore edilmekte, hastaların çiğneme etkinliği ve psikolojik adaptasyonu kalıcı olarak optimize edilmektedir.

Mastication is one of the fundamental functions of the stomatognatichal system, which can be impaired by the loss of natural teeth, and this condition is compensated through prosthetic dentistry. Today, implant-supported prostheses have become a frequently preferred reliable alternative to conventional complete dentures to improve patients' masticatory function and quality of life. Although peripheral feedback pathways are severely damaged by the loss of periodontal mechanoreceptors after tooth extraction, a special sensory perception called "osseoperception" develops over time in patients receiving implant therapy. Osseoperception is defined as the conscious perception of external stimuli transmitted by bone-supported prostheses through the activation of nerve endings located in the bone, periosteum, muscles, and joints around the implant. Histological and neurophysiological studies show that new nerve fibers can regenerate in the bone tissue around the implant after loading, and micro-deformations caused by the direct transfer of force to the bone stimulate these receptors. Although the active tactile threshold values of implants are higher than those of natural teeth, implant support increases tactile sensitivity and biting force approximately threefold compared to complete dentures, bringing them closer to the values of natural teeth and providing neuromuscular adaptation. Consequently, sensory-motor functions are partially restored through peri-implant innervation and the plasticity of the central nervous system, permanently optimizing patients' masticatory efficiency and psychological adaptation.
