Parafonksiyonel Alışkanlıklara Sahip Hastalarda Estetik Yaklaşımlar

Özet

Parafonksiyonel alışkanlıklar, özellikle diş sıkma ve bruksizm, aşırı oklüzal kuvvetler nedeniyle diş dokusuna zarar vererek, TME rahatsızlıklarına yol açarak ve restorasyonlarda kırılma veya aşınma gibi başarısızlıklar meydana getirerek diş hekimliğinde büyük endişe kaynağı oluşturmaktadır. Bu tür hastalarda uzun ömürlü, başarılı, estetik ve fonksiyonel rehabilitasyonlar sağlamak için doğru dental materyal seçimi ve tedavi planlaması kritik öneme sahiptir. Gelişen teknolojiyle birlikte tam seramik sistemler ve CAD/CAM uygulamaları geleneksel yöntemlere güçlü alternatifler sunmakta, porselen laminate veneerler, cam seramikler (lityum disilikat), polimer infiltre seramikler (PICN) ve PEEK gibi yüksek performanslı polimerler estetik beklentileri karşılamada önemli roller üstlenmektedir. Bununla birlikte, bruksizm hastalarında oklüzal splint kullanımı ve özellikle yapısal komplikasyon riskini en aza indiren, aşınma değerleri doğal diş yapısıyla uyumlu, yüksek bükülme direncine sahip monolitik zirkonyum dioksit kronların tercih edilmesi tedavi prognozunu ve öngörülebilirliğini önemli ölçüde iyileştirmektedir. Sonuç olarak, parafonksiyonel alışkanlığı olan bireylerde restorasyon tipi ve materyali belirlenirken hastanın estetik beklentileri, bruksizmin şiddeti ve tolerans limitleri gibi çoklu faktörler bir arada değerlendirilmelidir.

Parafunctional habits, especially tooth clenching and bruxism, cause great concern in dentistry by damaging dental tissues due to excessive occlusal forces, leading to TMD disorders, and causing failures such as fracture or wear in restorations. In such patients, the correct choice of dental materials and treatment planning are of critical importance to achieve long-lasting, successful, aesthetic, and functional rehabilitations. Along with advancing technology, all-ceramic systems and CAD/CAM applications offer strong alternatives to conventional methods, while porcelain laminate veneers, glass-ceramics (lithium disilicate), polymer-infiltrated ceramics (PICN), and high-performance polymers like PEEK play significant roles in meeting aesthetic expectations. However, the use of occlusal splints and particularly the preference for monolithic zirconia crowns, which exhibit high flexural strength, minimize the risk of structural complications, and possess wear values compatible with natural tooth structure, significantly improve the treatment prognosis and predictability in bruxism patients. In conclusion, when selecting the restoration type and material for individuals with parafunctional habits, multiple factors such as the patient's aesthetic expectations, the severity of bruxism, and tolerance limits must be evaluated together.
