Çocuk Hastalarda Dental Radyografi

Özet

Dental radyografik yöntemler teşhis, tedavi planlaması ve takip amacıyla diş hekimliğinde uzun yıllardır kullanılmakta, ancak bu yöntemlerin temel çalışma prensibi olan iyonize radyasyon, çocukların yüksek mitotik aktivite göstermesi ve az gelişmiş hücre yapıları nedeniyle onlar için daha yüksek bir kanser ve doku hasarı riski taşımaktadır. Çocuklarda çürük teşhisi, sürme problemleri, travma ve büyüme-gelişmenin değerlendirilmesi gibi çeşitli endikasyonlarla periapikal, bitewing, okluzal gibi intraoral; sefalometrik, panoramik ve üç boyutlu görüntüleme sağlayan konik ışınlı bilgisayarlı tomografi (KIBT) gibi ekstraoral yöntemler kullanılmaktadır. Ancak KIBT gibi yüksek radyasyon dozuna sahip tekniklerin rutin kullanımı uygun değildir. Radyasyondan korunmada temel ilkeler tetkikin gerekçelendirilmesi ve optimizasyon olup, süreç zamanla "Mümkün olan en düşük doz" (ALARA) prensibinden, endikasyona ve hastaya özel teşhis düzeyini hedefleyen ALADAIP prensibine evrilmiştir. Çocuklarda gereksiz maruziyeti önlemek için hekimin hastanın tıbbi öyküsü ve ağız sağlığına göre minimum sayıda tetkik istemesi, koruyucu ekipman kullanması ve çekim esnasında film tekrarlarını azaltacak davranış yönlendirme tekniklerinden faydalanması büyük önem taşımaktadır. Sonuç olarak, çocuklarda dental radyografi değerli bir tanı aracı olsa da tanısal fayda ile radyasyon riskleri dikkatle tartılmalı ve doz hastaya özel optimize edilmelidir.

Dental radiographic methods have been used in dentistry for many years for diagnosis, treatment planning, and follow-up; however, ionized radiation, which is the basic working principle of these methods, carries a higher risk of cancer and tissue damage for children due to their high mitotic activity and less developed cell structures. Various intraoral methods such as periapical, bitewing, and occlusal, alongside extraoral methods like cephalometric, panoramic, and cone-beam computed tomography (CBCT)—which provides three-dimensional imaging—are utilized for children under several indications including caries diagnosis, eruption problems, trauma, and growth evaluation. However, the routine use of techniques with high radiation doses like CBCT is not appropriate. The fundamental principles of radiation protection are justification and optimization of the examination, and this process has evolved over time from the "As Low As Reasonably Achievable" (ALARA) principle to the ALADAIP principle, which aims for an indication-oriented and patient-specific diagnostic level. To prevent unnecessary exposure in children, it is of great importance for the dentist to request the minimum number of examinations based on the patient's medical history and oral health, utilize protective equipment, and benefit from behavior guidance techniques that reduce repeated films during exposure. Consequently, although dental radiography is a valuable diagnostic tool in children, the diagnostic benefit must be carefully weighed against radiation risks, and the dose must be optimized specifically for the patient.
