Özet

Yaşa bağlı maküla dejenerasyonu (YBMD), gelişmiş toplumlarda yaşlı nüfusta kalıcı körlüğün başlıca nedenlerinden biri olan, makülanın harabiyetiyle seyreden ilerleyici bir göz hastalığıdır. Klinik olarak kuru ve eksudatif (yaş) olmak üzere iki tipi bulunan hastalık; erken evrelerde kırıklı görme ve görme keskinliğinde azalma yaparken, ileri evrelerde merkezi görme kaybına yol açarak bireylerin günlük yaşam aktivitelerini ve bağımsızlıklarını olumsuz etkiler. En önemli risk faktörü ilerleyen yaş olmakla birlikte, sigara kullanımı, sistemik hipertansiyon, genetik yatkınlık, artmış vücut kütle indeksi ve etnik köken gibi sosyodemografik, oküler ve çevresel faktörler de hastalığın etiyolojisinde rol oynar. Patogenezinde retina pigment epiteli ve bruch membranının yaşlanması temel alınır; bu süreç retina altındaki sarı birikintiler olan drusen oluşumuyla başlar. Teşhiste dilatasyonlu fundus muayenesi ve optik koherens tomografi (OCT) kritik öneme sahiptir. Erken evredeki olgularda, hastalığın ilerlemesini yavaşlatmak amacıyla AREDS 2 kriterlerine uygun omega 3, lutein ve çeşitli vitaminleri içeren nutrisyonel oral suplement tedavileri uygulanır. Eksudatif tipte gelişen koroidal neovasküler membran varlığında ise intravitreal bevacizumab, ranibizumab ve aflibercept gibi monoklonal antikor ajanları kullanılmaktadır. Hastalığın erken evrede yakalanıp kalıcı görme kayıplarının önlenmesi için 60 yaş ve üzerindeki bireylerin yılda bir kez ayrıntılı göz muayenesi yaptırması önerilmektedir.

Age-related macular degeneration (AMD) is a progressive eye disease characterized by macular degeneration, which is one of the leading causes of irreversible blindness among the elderly in developed societies. Clinically divided into dry and exudative (wet) types, the disease causes distorted vision and decreased visual acuity in its early stages, while leading to central vision loss in advanced stages, thereby negatively affecting individuals' daily life activities and their independence. Although the most significant risk factor is advancing age, sociodemographic, ocular, and environmental factors such as smoking, systemic hypertension, genetic predisposition, increased body mass index, and ethnicity also play a role in its etiology. Its pathogenesis is based on the aging of the retinal pigment epithelium and Bruch's membrane; this process begins with the formation of drusen, which are yellow subretinal deposits. Dilated fundus examination and optical coherence tomography (OCT) are critical for diagnosis. For early-stage cases, nutritional oral supplement therapies containing omega-3, lutein, and various vitamins are administered in accordance with AREDS 2 to slow disease progression. In the presence of choroidal neovascular membrane developing in the exudative type, monoclonal antibody agents such as intravitreal bevacizumab, ranibizumab, and aflibercept are utilized. To detect the disease early and prevent permanent vision loss, individuals aged 60 and older are recommended to undergo a detailed ophthalmological examination once a year.
