Oftalmik Viskoelastikler (OVD)
Özet
Oftalmik viskoelastik maddeler (OVD), oftalmoloji cerrahisinde daha güvenilir ve kolay tekniklerin kullanılmasına olanak tanıyan ajanlardır. İlk olarak sodyum hyalüronat ile başlayan bu süreç, özellikle katarakt cerrahisinde kornea hasarını ve ödemini azaltmada kritik bir rol oynamıştır. Bu maddelerin viskozite, elastisite, kohezivite, dispersivite, psödoplastisite ve viskoadaptivite gibi fiziksel özellikleri, cerrahi sırasındaki işlevlerini belirler. Örneğin, koheziv maddeler ön kamarayı korurken ve lens implantasyonunu kolaylaştırırken, dispersif maddeler endotel koruması sağlar. İdeal bir viskoelastik; optik olarak şeffaf, nontoksik, noninflamatuar, nonimmunojenik ve steril olmalıdır. Günümüzde katarakt, glokom, retina ve şaşılık cerrahilerinde sodyum hyalüronat, poliakrilamid, kondroitin sülfat ve hidroksipropil metilselüloz (HPMC) gibi farklı formülasyonlar kullanılmaktadır. Ancak bu maddelerin cerrahi sonrası geçici intraoküler basınç artışı, kapsüler blok sendromu, psödo-anterior üveit ve korneal ödem gibi önemli komplikasyonları bulunabilir. Bu nedenle OVD’lerin özelliklerinin ve doğru kullanım zamanlarının cerrah tarafından iyi bilinmesi, cerrahi başarıyı doğrudan artırır.
Ophthalmic viscoelastic devices (OVD) are agents that enable more reliable and easier techniques in ophthalmology surgery, significantly reducing corneal damage and postoperative edema, particularly during intraocular lens placement. The physical properties of these substances—such as viscosity, elasticity, cohesiveness, dispersiveness, pseudoplasticity, and viscoadaptivity—determine their intraoperative functions. While cohesive OVDs maintain the anterior chamber and facilitate lens implantation, dispersive OVDs offer essential protection for the corneal endothelium. An ideal viscoelastic substance must be optically clear, non-toxic, non-inflammatory, non-immunogenic, and sterile. Today, various formulations such as sodium hyaluronate, polyacrylamide, chondroitin sulfate, and hydroxypropyl methylcellulose (HPMC) are widely utilized in cataract, glaucoma, retina, and strabismus surgeries. However, their use can lead to notable complications, including a transient spike in postoperative intraocular pressure, capsular block syndrome, pseudo-anterior uveitis, and corneal edema. Consequently, a comprehensive understanding of each agent's specific properties and correct application timings by the surgeon directly enhances surgical success rates.
