Özet

Dental anksiyete, dünya genelinde yaygın olarak görülen ve bireylerin ağız-diş sağlığını doğrudan olumsuz etkileyen karmaşık bir psikososyal problemdir. Korku belirli bir dış tehdide karşı gelişirken, anksiyete içsel ve belirsiz bir endişe halidir; dental anksiyete ise kişinin diş tedavisi beklentisiyle veya koltuğa oturduğu an hissettiği yoğun kaygıyı ifade eder. Kadınlarda, çocuklarda ve adölesanlarda daha sık görülen bu durumun etiyolojisinde geçmiş travmatik deneyimler, ağrı beklentisi, klinikteki alet sesleri, kokular, kontrol kaybı hissi ve ailevi yönlendirmeler yer almaktadır. Bu psikolojik uyarılma tükürük akışını azaltarak çürük riskini artırır; yüksek anksiyeteli bireyler hijyeni ihmal etmeye, yanlış beslenmeye ve randevularını ertelemeye meyillidir. Tedaviden kaçınma davranışı, basit sorunların karmaşık klinik tablolara dönüşmesine ve dolayısıyla bireyin yaşam kalitesinin düşmesine yol açan kısır bir döngü yaratır. Problemin yönetiminde Corah (C-DAS) veya Modifiye (MDAS) gibi anksiyete ölçekleriyle erken teşhis kritik önem taşır. Diş hekimleri, hastaların endişelerini gidermek ve etkin tedavi sunabilmek amacıyla oral, intravenöz sedasyon veya genel anestezi gibi farmakolojik yöntemlerin yanı sıra iatrosedasyon, hipnoz ve bilişsel davranışsal terapi gibi psikolojik yaklaşımları entegre ederek multidisipliner bir strateji uygulamalıdır.

Dental anxiety is a common and complex psychosocial problem worldwide that directly and adversely affects individuals' oral and dental health. While fear develops against a specific external threat, anxiety is an internal and uncertain state of apprehension; dental anxiety refers to the intense apprehension a person feels in anticipation of dental treatment or the moment they sit in the chair. This condition, more prevalent in women, children, and adolescents, stems from an etiology including past traumatic experiences, pain expectations, clinic sounds, odors, perceived lack of control, and familial conditioning. This psychological arousal increases caries risk by reducing salivary flow; highly anxious individuals are prone to neglecting hygiene, dietary changes, and delaying appointments. Avoidance behavior creates a vicious cycle where simple issues turn into complex clinical situations, thereby reducing the individual's quality of life. Early diagnosis with anxiety scales like Corah (C-DAS) or Modified (MDAS) is critical in management. Dentists should implement a multidisciplinary strategy, integrating pharmacological methods such as oral, intravenous sedation, or general anesthesia, alongside psychological approaches like iatrosedation, hypnosis, and cognitive behavioral therapy to alleviate patient concerns and provide effective treatment.
