Parameniskal Kistlerde Güncel Tedavi Yaklaşımları

Özet

Parameniskal kistler, menisküsün periferik zonunda yer alan ve çoğunlukla menisküs yırtıklarının eşlik ettiği kistik lezyonlardır. Etyolojisinde miksoid dejenerasyon veya eklem sıvısının dışarı çıkmasına neden olan tek yönlü bir valf mekanizması suçlanmaktadır. Tanıda altın standart MR görüntülemesi olup; hastalarda ağrı, kilitlenme, takılma ve fleksiyonda belirginleşen Pisani bulgusu gibi semptomlar görülür. Tedavi planlaması kistin boyutu, eşlik eden menisküs patolojisi ve hastanın şikayetlerine göre konservatif veya cerrahi olarak belirlenir. Cerrahi tedavide temel amaç kistin ortadan kaldırılması ve meniskal patolojinin aynı seansta giderilmesidir; bu doğrultuda açık kistektomi veya artroskopik dekompresyon yöntemleri tercih edilmektedir. Son yıllarda, klasik cerrahi yaklaşımların getirdiği geniş skar izleri ve klasik artroskopide karşılaşılan erişim zorlukları ile rekürrens (tekrarlama) risklerini azaltmak amacıyla çeşitli modifiye teknikler geliştirilmiştir. Ekstra superomedial/superolateral portaller, submeniskal portaller veya çoklu portalli endoskopik yaklaşımlar sayesinde, özellikle anterior menisküsteki lezyonlara ve multiloküle dev kistlere daha güvenli ve etkili bir şekilde müdahale edilebilmektedir. Güncel literatür ışığında, kistin boyutuna, yerleşim yerine ve altta yatan menisküs yırtığının durumuna göre en uygun yöntemin seçilmesi tedavi başarısını anlamlı ölçüde artırmaktadır.

Parameniscal cysts are cystic lesions located in the peripheral zone of the meniscus, mostly accompanied by meniscal tears. In its etiology, myxoid degeneration or a one-way valve mechanism causing the escape of joint fluid is implicated. MRI is the gold standard for diagnosis, and patients present with symptoms such as pain, locking, catching, and the Pisani sign, which becomes prominent in flexion. Treatment planning is determined conservatively or surgically based on cyst size, accompanying meniscus pathology, and the patient's complaints. The primary goal in surgical treatment is the elimination of the cyst and management of the meniscal pathology in the same session; accordingly, open cystectomy or arthroscopic decompression methods are preferred. In recent years, various modified techniques have been developed to reduce the extensive scar tissue caused by open surgery, access difficulties in classical arthroscopy, and recurrence risks. Thanks to extra superomedial/superolateral portals, submeniscal portals, or multi-portal endoscopic approaches, interventions for lesions in the anterior meniscus and multi-loculated giant cysts can be performed more safely and effectively. In light of current literature, selecting the most appropriate method according to the size, location of the cyst, and the underlying meniscal tear significantly increases treatment success.
