Özet

Diabetes Mellitus, insülin eksikliği veya etkisizliği sonucu gelişen ve sürekli tıbbi bakım gerektiren kronik bir metabolik hastalıktır. Temel olarak β hücre hasarıyla karakterize Tip 1 ve insülin direnci ile salgılanma bozukluğuna dayanan Tip 2 diyabet olmak üzere ikiye ayrılır. Cerrahi müdahale görecek hastaların yaklaşık %25'ini diyabetliler oluşturmaktadır. Ameliyat dönemi stres ve açlık gibi faktörler glikoz metabolizmasını olumsuz etkilediğinden, perioperatif süreçte hiperglisemi ve hipoglisemi risklerini önlemek adına kan şekeri regülasyonunun iyi yapılması, morbidite ve mortalite oranlarının düşürülmesi için hayati önem taşır. Glikoz-insülin-potasyum (GİK) solüsyonu veya ayrı yol insülin infüzyonu gibi protokoller kullanılarak glisemik kontrol sağlanır. Ayrıca obezite, visseral yağlanma ve kortizol artışı vasıtasıyla insülin direncini tetikleyerek Tip 2 diyabet gelişiminde önemli bir rol oynar. Obez bireylerde yaşam tarzı değişiklikleri veya sleeve gastrektomi ve Roux-en-y gastrik baypas gibi obezite cerrahisi yöntemleriyle sağlanacak kalıcı kilo kayıpları, insülin duyarlılığını artırarak kan glikoz düzeylerinde ve HbA1c seviyelerinde belirgin iyileşmeler sağlamaktadır. Hem ameliyat öncesi hazırlık hem de taburculuk sonrası süreçte hasta ve yakınlarının eğitimi başarı şansını artırır.

Diabetes Mellitus is a chronic metabolic disease that requires continuous medical care, developing as a result of insulin deficiency or ineffectiveness, and is basically divided into Type 1, characterized by beta cell damage, and Type 2, based on insulin resistance and secretion disorders. Diabetic patients constitute approximately 25% of the patients who will undergo surgical intervention, and since factors such as surgical stress and fasting adversely affect glucose metabolism, maintaining good blood sugar regulation in the perioperative period to prevent hyperglycemia and hypoglycemia risks is of vital importance for reducing morbidity and mortality rates. Glycemic control is achieved using protocols such as glucose-insulin-potassium (GIK) solution or separate-route insulin infusion. Furthermore, obesity plays a significant role in the development of Type 2 diabetes by triggering insulin resistance through visceral adiposity and increased cortisol. Sustained weight loss achieved in obese individuals through lifestyle changes or bariatric surgery methods like sleeve gastrectomy and Roux-en-y gastric bypass increases insulin sensitivity, leading to significant improvements in blood glucose and HbA1c levels, while the education of patients and their relatives in both the preoperative preparation and post-discharge processes enhances the success rate of diabetes management.
