Özet

Yenidoğan döneminde inme, arteriyel veya venöz tromboz kaynaklı serebral kan akımı bozukluklarını kapsayan, yüksek mortalite ve morbidite riski taşıyan bir hastalık grubudur. En yaygın tip olan akut semptomatik perinatal arteriyel iskemik inme (ASPAİİ) genellikle orta serebral arter tıkanıklığı ile seyrederken; varsayılan perinatal iskemik inme, periventriküler venöz inme, neonatal hemorajik inme ve serebral sinovenöz tromboz gibi diğer türler kendine özgü patofizyolojik mekanizmalarla gelişir. Tanı sürecinde kranial MRG en güvenilir görüntüleme yöntemi olup, yönetim genellikle nöbet kontrolü, hayati fonksiyonların desteklenmesi ve gerektiğinde antikoagülan tedavisi gibi destekleyici yaklaşımları içerir. Bu durumlar, uygun tanı ve erken tedavi edilmediklerinde ciddi nörolojik sekellere yol açabilir.

 

Neonatal stroke encompasses a heterogeneous group of disorders caused by arterial or venous thrombosis, significantly contributing to neonatal mortality and long-term morbidity. While acute symptomatic perinatal arterial ischemic stroke (ASPAİİ) is the most frequent subtype, typically involving large vessel occlusion, other forms like presumed perinatal ischemic stroke, periventricular venous stroke, neonatal hemorrhagic stroke, and cerebral sinovenous thrombosis (SSVT) present with distinct clinical and etiological characteristics. Cranial MRI serves as the definitive diagnostic tool, and management relies on supportive care, seizure control, and selective use of anticoagulants based on the underlying condition. Prompt diagnosis and intervention are critical to mitigate severe long-term neurodevelopmental sequelae.
