Yenidoğanlarda Davranışsal Organizasyon ve Bireyselleştirilmiş Destekleyici Gelişimsel Bakım
Özet
Yenidoğan yoğun bakım ünitelerinde (YDYBÜ) takip edilen preterm bebeklerin nörogelişimsel ve optimal organizasyonunu desteklemeyi amaçlayan Bireyselleştirilmiş Destekleyici Gelişimsel Bakım Programı (NIDCAP), temelini yenidoğan davranışlarını beş alt sistemde inceleyen Sinaktif Teori'den almaktadır. Preterm bebekler, YDYBÜ'deki gürültü, yüksek ışık ve ağrılı işlemler gibi çevresel stresörler nedeniyle büyüme geriliği ve uzun dönemde bilişsel/sosyal dezavantajlar yaşayabilmektedir. Bu olumsuz etkileri en aza indirmek adına ses ve ışık kontrolünü içeren iyileştirici çevre, güvenli uyku, aile merkezli bakım ve gelişimsel destekleyici aktiviteler (besleyici olmayan emme, fleksiyon postürünü koruyan yuva pozisyonu, kanguru bakımı) temel bileşenler olarak uygulanmaktadır. Nörolojik gelişimin hızlandığı bu hassas dönemde, hemşirelik girişimlerinin minimal temas ve gece-gündüz örüntüsüne göre planlanması kritik önem taşır. Detaylandırılan olgu sunumunda ise, 32+9 haftalık ikiz eşi olarak doğan, prematüre ve intrauterin gelişme geriliği (IUGR) nedeniyle YDYBÜ'de tedavi gören "Bebek Y"nin klinik durumu; Marjory Gordon'un Fonksiyonel Sağlık Örüntüleri Modeli'ne göre fiziksel muayene, laboratuvar sonuçları ve uygulanan tedavilerle birlikte incelenmiştir.
The Newborn Individualized Developmental Care and Assessment Program (NIDCAP), which aims to support the neurodevelopmental and optimal organization of preterm infants in neonatal intensive care units (NICUs), is based on the Synactive Theory that examines newborn behaviors across five subsystems. Preterm infants can experience growth retardation and long-term cognitive or social disadvantages due to environmental stressors in the NICU, such as noise, bright light, and painful procedures. To mitigate these negative effects, core components are implemented: a healing environment including sound and light control, safe sleep, family-centered care, and developmentally supportive activities like non-nutritive sucking, therapeutic positioning supporting flexion posture, and kangaroo care. During this sensitive period of rapid neurological development, planning nursing interventions based on minimal touch and day-night patterns is of critical importance. In the detailed case report, the clinical status of "Baby Y," a 32+9 week twin born with prematurity and intrauterine growth restriction (IUGR) treated in the NICU, is examined through physical examination, laboratory results, and applied treatments according to Marjory Gordon's Functional Health Patterns Model.
