Cilt Sorunu Olan Yenidoğan ve Hemşirelik Bakımı
Özet
Yenidoğan bebeklerin anatomik ve fizyolojik cilt yapısını, gelişim özelliklerini ve kanıta dayalı hemşirelik bakımı uygulamalarını ele alan bu çalışma, cildin termoregülasyon, sıvı dengesi ve enfeksiyon bariyeri gibi hayati fonksiyonlarını açıklamaktadır. Term ve preterm yenidoğanlarda morfolojik ve fonksiyonel açıdan belirgin farklılıklar gösteren cilt yapısı, özellikle preterm bebeklerde stratum korneum tabakasının yetersiz olgunlaşması nedeniyle enfeksiyon riski, transepidermal sıvı kayıpları, toksisite ve hipotermiye eğilimi önemli ölçüde artırmaktadır. Güncel AWHONN kılavuzu temel alınarak cildin objektif değerlendirilmesi, güvenli banyo yönetimi, koruyucu verniks kazeozanın önemi, göbek ve sünnet bakımı, dezenfektan kullanımı, diaper dermatit önlenmesi, flaster yaralanmaları, nemlendirici uygulamaları ile intravenöz infiltrasyon ve ekstravazasyonların takibi gibi kritik klinik konular detaylandırılmaktadır. Son bölümde ise, dermo-epidermal bağların eksikliğinden kaynaklanan ve hafif mekanik travmalarla bül oluşumuyla karakterize kalıtsal bir hastalık olan Epidermolysis Bullosa Simpleks tanılı dört günlük bir erkek bebeğin hemşirelik süreci ve Gordon’un Fonksiyonel Sağlık Örüntüleri’ne göre değerlendirildiği olgu sunumu paylaşılmıştır. Yenidoğan yoğun bakım hemşirelerinin, bu hassas süreçlerde enfeksiyonların önlenmesi, yara yönetimi, multidisipliner ekip çalışması ve aileye verilen taburculuk eğitimi üzerindeki hayati rolü önemle vurgulanmaktadır.
Focusing on the anatomical and physiological skin structure, developmental characteristics, and evidence-based nursing care practices of newborn infants, this study explains the vital functions of the skin such as thermoregulation, fluid balance, and serving as an infection barrier. The skin structure displays distinct morphological and functional differences between term and preterm neonates; especially in preterm infants, the inadequate maturation of the stratum corneum significantly increases the risk of infection, transepidermal water loss, toxicity, and susceptibility to hypothermia. Based on the current AWHONN guideline, critical clinical topics such as objective skin assessment, safe bathing management, the importance of the protective vernix caseosa, umbilical cord and circumcision care, disinfectant use, diaper dermatitis prevention, medical adhesive injuries, emollient applications, and the monitoring of intravenous infiltration and extravasation are detailed. The final section presents a case report of a four-day-old male infant diagnosed with Epidermolysis Bullosa Simplex, a hereditary disease caused by the lack of dermo-epidermal junctions and characterized by blister formation due to minor mechanical trauma, evaluated according to Gordon's Functional Health Patterns. The vital role of neonatal intensive care nurses in preventing infections, wound management, multidisciplinary teamwork, and discharge education for the family during this sensitive process is strongly emphasized.
