Yenidoğanlarda Solunum Sistem Sorunları ve Hemşirelik Bakımı
Özet
Yenidoğan döneminde karşılaşılan solunum sistemi sorunları, bebeklerin intrauterin yaşamdan ekstrauterin yaşama geçiş süreçleri ve bu kritik evrede uygulanan klinik hemşirelik bakımı metinde kapsamlı bir şekilde incelenmektedir. Doğum sonrasında solunumun sağlıklı bir şekilde başlaması yenidoğanın hayatta kalması için en temel faktördür; bu doğrultuda alveolar yüzey gerilimini azaltan sürfaktan maddesinin rolü ve fetal akciğer sıvısının uzaklaştırılması mekanizmaları detaylıca açıklanmaktadır. Prematüre bebeklerde gelişen Respiratuar Distres Sendromu (RDS), fetal sıvının emilememesiyle oluşan Yenidoğan Geçici Takipnesi (YGT), mekonyumlu amniyotik sıvının aspire edilmesiyle meydana gelen Mekonyum Aspirasyon Sendromu (MAS), kronik akciğer hasarını ifade eden Bronkopulmoner Displazi (BPD) ve solunumun durması olan Apne hastalıklarının klinik bulguları, tedavi protokolleri ve hemşirelik yönetimleri ele alınmaktadır. Ayrıca koanal atrezi, larinks obstrüksiyonları ve Pierre Robin sendromu gibi konjenital anomalilere yer verilmektedir. Son bölümde, 30. gebelik haftasında 1500 gram olarak doğan ve RDS tanısıyla yoğun bakıma yatırılan "Bebek S" olgusu sunulmuştur. Bu bebek ve endişeli ailesi, Marjory Gordon'un Fonksiyonel Sağlık Örüntüleri Modeline göre sistematik olarak değerlendirilerek gaz değişimi, etkisiz termoregülasyon, beslenme yetersizliği ve ebeveyn-bebek bağlılığı gibi hemşirelik tanıları, girişimleri ve değerlendirme sonuçları üzerinden analiz edilmektedir.
Respiratory system problems encountered in the neonatal period, the adaptation process of newborns from intrauterine to extrauterine life, and the clinical nursing care implemented during this phase are comprehensively examined in the text. The healthy initiation and maintenance of respiration after birth is the most critical factor for newborn survival; accordingly, the role of surfactant in reducing alveolar surface tension and the mechanisms of clearing fetal lung fluid are explained in detail. The text details the clinical findings, treatment protocols, and nursing management for major disorders, including Respiratory Distress Syndrome (RDS) caused by surfactant deficiency in premature infants, Transient Tachypnea of the Newborn (TTN) resulting from inadequate fluid absorption, Meconium Aspiration Syndrome (MAS) due to aspiration of meconium-stained amniotic fluid, Bronchopulmonary Dysplasia (BPD) as a chronic lung injury, and Apnea defined as cessation of breathing. Furthermore, congenital anomalies obstructing respiration, such as choanal atresia, laryngeal obstructions, and Pierre Robin syndrome, are included. In the final section, a case study of "Baby S," who was born at 30 weeks of gestation weighing 1500 grams and admitted to the intensive care unit with RDS, is presented. This infant and the anxious family are systematically evaluated according to Marjory Gordon's Functional Health Patterns Model, analyzing nursing diagnoses, interventions, and evaluation results in areas such as gas exchange, ineffective thermoregulation, nutritional deficiency, and parent-infant attachment.
