Özet

Bu kitap bölümü, dünya çapındaki kurumsal ortamlarda normal fizyolojik doğumun giderek yok olmasını ve bunu anlamanın ile desteklemenin zorluklarını ele almaktadır. Ebeler doğumun her aşamasına tanık olurken, doktorların fizyolojik doğumu düzenli olarak görmemesi doğal sürece güvenmelerini zorlaştırmakta ve onları aşırı riskten kaçınma eğilimine itmektedir. Tüm gebeliklerin yüksek riskli kabul edilerek müdahale edilmesi, gerçek patoloji sergileyenlerin gözden kaçması riskini doğurmaktadır. Kitabın ilk bölümlerinde otörler, normal doğumun doğasını, biyolojik ve fizyolojik nedenlerini vurgulayarak sağlıklı anne ve bebekleri gereksiz müdahalelerden koruma ihtiyacını savunmaktadır. Bakımda salutojenik yaklaşımların riski azaltma ile dengelenmesi, insan haklarına saygılı ve kadın merkezli bir kültürün oluşturulması önerilmektedir. Psikolojik, duygusal ve fizyolojik durumların birbirine bağlılığı ile doğum yapılan fiziksel alanın tasarımı ve çalışanların tutumlarının sonuçlar üzerindeki etkisi derinlemesine araştırılmaktadır. Sonuç olarak, doğum bakımını sağlayanların fizyolojik süreçlerin tanınması ile gerçek patolojinin tedavisi arasında bir denge kurması, anneler ve bebekler için hem güvenliği hem de olumlu refahı sağlayacaktır.

This book chapter addresses the gradual disappearance of normal physiological birth in institutional settings worldwide and the challenges of understanding and supporting it. While midwives witness all stages of childbirth, doctors do not regularly see physiological birth, which makes it difficult for them to trust the natural process and drives them toward excessive risk aversion. Treating and intervening in all pregnancies as high-risk poses the risk of overlooking those exhibiting real pathology. In the initial chapters, the authors advocate for the need to protect healthy mothers and babies from unnecessary interventions by emphasizing the nature, biological, and physiological reasons of normal birth. It is suggested that salutogenic approaches in care should be balanced with risk reduction, creating a woman-centered culture respectful of human rights. The interconnectedness of psychological, emotional, and physiological states, along with the design of the physical birthing space and the attitudes of staff, and their impact on outcomes are thoroughly explored. Consequently, it is imperative for birth care providers to establish a balance between recognizing physiological processes and treating actual pathology, which will ensure both safety and positive well-being for mothers and babies.
