Özet

Bu çalışma, doğum öncesi ve doğum sırasındaki faktörlerin yenidoğanın epigenetik mekanizmaları üzerindeki etkilerini ve bunun uzun vadeli sağlık sonuçlarıyla ilişkisini incelemektedir. David Barker tarafından ortaya konan Sağlığın ve Hastalığın Gelişimsel Kökenleri (DOHaD) hipotezi, erken yaşam ortamının yetişkinlikteki kronik hastalık riskini belirlediğini savunmaktadır. Epigenetik süreçler, özellikle DNA metilasyonu, bu programlamanın temel biyolojik mekanizmalarından biri olarak kabul edilmektedir. Araştırmalar; sezaryen doğum, doğum indüksiyonu ve sentetik oksitosin kullanımı gibi müdahalelerin kordon kanındaki DNA metilasyon paternlerini değiştirdiğini ve çocukları astım, Tip 1 diyabet, obezite gibi bulaşıcı olmayan kronik hastalıklara yatkın hale getirebildiğini göstermektedir. EPIIC hipotezi ise fizyolojik ve müdahalesiz doğum süreçlerinin anne ve bebek üzerinde "östres" (sağlıklı stres) yaratarak optimal epigenetik çalıştırmayı desteklediğini ileri sürer. Küresel düzeyde gereksiz tıbbi müdahalelerin katlanarak arttığı günümüzde, müdahalelerin kısa ve uzun vadeli nörohormonal ile epigenetik etkilerinin net olarak anlaşılması, "öncelikle zarar verme" ilkesi doğrultusunda büyük bir aciliyet taşımaktadır. Bu nedenle, kanıta dayalı ve fizyolojik süreçlere güvenen bakım modellerinin geliştirilmesi önem arz etmektedir.

This study examines the effects of prenatal and intrapartum factors on the epigenetic mechanisms of the newborn and their relationship with long-term health outcomes. The Developmental Origins of Health and Disease (DOHaD) hypothesis, introduced by David Barker, posits that the early life environment determines the risk of chronic diseases in adulthood. Epigenetic processes, particularly DNA methylation, are recognized as one of the fundamental biological mechanisms mediating this programming. Research indicates that interventions such as cesarean section, labor induction, and the use of synthetic oxytocin alter DNA methylation patterns in cord blood, potentially predisposing children to non-communicable chronic diseases like asthma, Type 1 diabetes, and obesity. Conversely, the EPIIC hypothesis suggests that physiological and unassisted birth processes exert a form of "eustress" (healthy stress) on the mother and infant, thereby enabling optimal epigenetic triggering. In an era where unnecessary medical interventions are exponentially increasing globally, a comprehensive understanding of the short- and long-term neurohormonal and epigenetic impacts of these interventions is of utmost urgency in accordance with the principle of "first do no harm". Consequently, developing models of care that are evidence-based and place trust in physiological processes remains crucial.
