Özet

Günümüzdeki gelişmiş görüntüleme yöntemleri, retina anatomisinde yeni tanımlamaların doğmasını sağlamıştır. Işık mikroskobunda 10 histolojik tabakadan oluşan retina, iç limitan membrandan başlayarak retina pigment epiteline kadar uzanır. Bu tabakalar arasında yer alan retina sinir lifi tabakası glokom teşhisinde, ganglion hücreleri ise görsel uyaranların iletiminde kritik öneme sahiptir. Optik Koherens Tomografi (OKT), retinanın enine kesitlerini invaziv olmayan bir şekilde inceleyen, derinlik çözünürlüğü ve penetrasyonu ile oftalmolojide devrim yaratmış bir teknolojidir. Zaman içinde time-domain, spektral-domain ve swept-source gibi daha hızlı ve yüksek çözünürlüklü OKT teknikleri geliştirilmiştir. OKT sayesinde merkezi retinada ve koroidde toplam 18 anatomik yapı tanımlanmış, fotoreseptörlerin iç segmentleri myoid ve elipsoid zon olarak ikiye ayrılmıştır. Gelişen OKT Anjiyografi (OKTA) teknolojisi ise, geleneksel floresein anjiyografinin aksine boya sızıntısı ve enjeksiyon gerektirmeden, retinanın yüzeysel, derin ve koriokapillaris gibi tüm vasküler katmanlarındaki kan akışını üç boyutlu ve yüksek çözünürlüklü olarak bağımsızca görüntüleme imkanı sunar. OKTA, sınırlı dinamik aralık ve artefakt riskine rağmen; diyabet, damar tıkanıklıkları ve yaşa bağlı makula dejenerasyonu gibi hastalıkların erken teşhisinde, takibinde ve tedavi kararlarında hekimlere ortak bir dil ve güncel bir isimlendirme standardı sağlayarak rehberlik etmektedir.

Advanced contemporary imaging methods have led to new definitions in retinal anatomy, which histologically consists of ten layers visible under light microscopy, extending from the inner limiting membrane to the retinal pigment epithelium. Among these, the retinal nerve fiber layer is crucial for glaucoma diagnosis, while ganglion cells are responsible for transmitting visual stimuli to the brain. Optical Coherence Tomography (OCT) is a non-invasive imaging technique providing cross-sectional retinal views that revolutionized ophthalmology, evolving through time-domain, spectral-domain, and swept-source technologies to deliver higher speeds and resolutions. Current OCT consensus identifies eighteen anatomic structures in the central retina and choroid, further dividing the photoreceptor inner segment into myoid and ellipsoid zones. Building upon this, OCT Angiography (OCTA) offers significant advantages over traditional fluorescein angiography by providing high-contrast, non-invasive, and dye-free three-dimensional blood flow visualization across distinct vascular layers, including the superficial vascular plexus, deep capillary plexus, and choriocapillaris. Despite functional limitations such as a limited dynamic range and vulnerability to motion artifacts, careful interpretation of OCTA imagery enables reliable detection of vascular changes in conditions like diabetes, vascular occlusions, and age-related macular degeneration, thereby guiding clinical prognosis, evaluating treatment efficacy, and establishing a standardized updated terminology among ophthalmologists.
