Özet

Kuru göz; gözyaşı filminin lipid, aköz ve müköz tabakalarından birinin veya birkaçının yetersizliği sonucu oluşan, artmış gözyaşı osmolaritesi ve oküler yüzey inflamasyonu ile karakterize bir hastalıktır. Yaş, kadın cinsiyet, hormonal değişimler, otoimmün hastalıklar, kontakt lens kullanımı ve dijital ekran ile klima gibi çevresel faktörler bu duruma zemin hazırlar. Temel olarak gözyaşı eksikliğine bağlı (aköz yetmezlik) ve buharlaşmanın artmasına bağlı (evaporatif) olmak üzere iki ana grupta sınıflandırılsa da hastaların büyük çoğunluğunda miks form görülür. Tanısında detaylı bir anamnezin yanı sıra Gözyaşı Filmi Parçalanma Zamanı (BUT), Schirmer testi ve oküler yüzey boyamaları gibi klinik testlerden yararlanılır. Hastalarda yanma, batma, yabancı cisim hissi ve bulanık görme gibi yaşam kalitesini düşüren semptomlara yol açar. Tedavinin temel amacını semptomları kontrol altına almak ve oküler yüzey hasarını önlemek oluşturur. Bu kapsamda en kritik ilk basamak hasta eğitimi olup; ortam havasının nemlendirilmesi, ekran başında göz kırpma sıklığının artırılması ve kapak hijyeni gibi uygulamaları içerir. Medikal tedavide ise eksikliği tamamlayan yapay gözyaşı preparatları, inflamasyonu baskılayan topikal siklosporin veya kısa süreli kortikosteroidler, mukolitik ajanlar ve otolog serumlar kullanılır. İleri ve dirençli vakalarda ise kontakt lensler ile punktum oklüzyonu veya tarsorafi gibi cerrahi seçenekler değerlendirilir.

Dry eye is a disease characterized by increased tear osmolarity and ocular surface inflammation resulting from the insufficiency of one or more of the lipid, aqueous, and mucous layers of the tear film. Age, female gender, hormonal changes, autoimmune diseases, contact lens use, and environmental factors such as digital screens and air conditioning predispose to this condition. Although basically classified into two main groups as aqueous deficiency and evaporative dry eye, the vast majority of patients exhibit a mixed form. In its diagnosis, clinical tests such as Tear Film Break-up Time (BUT), Schirmer test, and ocular surface staining are utilized alongside a detailed medical history. It causes symptoms that reduce the quality of life, such as burning, stinging, foreign body sensation, and blurred vision. The primary goal of treatment is to control symptoms and prevent ocular surface damage. The most critical first step in this context is patient education, which includes practices like humidifying the ambient air, increasing the blinking frequency in front of screens, and eyelid hygiene. In medical treatment, artificial tear preparations that supplement the deficiency, topical cyclosporine or short-term corticosteroids that suppress inflammation, mucolytic agents, and autologous serums are used. In advanced and resistant cases, contact lenses and surgical options such as punctal occlusion or tarsorrhaphy are evaluated.
