İridokorneal Endotelyal Sendromlar
Özet
İridokorneal endotelyal (İKE) sendromlar, genellikle tek bir gözü etkileyen ve çoğunlukla orta yaş kadınlarda görülen, korneal endotel tabakasının anormal proliferasyonu ile karakterize bir hastalık grubudur. Bu sendrom; Chandler sendromu, ilerleyici iris atrofisi ve Cogan-Reese sendromu olmak üzere üç alt başlıkta incelenir. Patogenezinde viral enfeksiyonların (özellikle Herpes simpleks ve Epstein-Barr) tetikleyici rol oynadığı düşünülmektedir. Hastalık, endotel hücrelerinin kontrolsüz çoğalarak ön kamara açısı ve irise doğru bir membran oluşturmasıyla periferik anterior sineşiye, iris değişikliklerine ve ikincil glokoma yol açar. Chandler sendromunda korneada "dövülmüş gümüş" görünümü ve belirgin kornea ödemi ön plandayken; ilerleyici iris atrofisinde korektopi, stromal incelme ve erime delikleri izlenir. Cogan-Reese sendromunda ise iris ön yüzeyinde karakteristik pigmentli nodüller ve iris mimarisinin kaybı belirgindir. Teşhiste speküler ve konfokal mikroskopi kritik öneme sahiptir. Glokom prevalansı %46-82 arasında değişmekte olup, medikal tedaviye dirençlidir. Tedavisinde trabekülektomi, glokom drenaj cihazları ve kornea dekompansasyonu için endotelyal keratoplasti (DMEK/DSAEK) teknikleri uygulanır, ancak uzun dönem prognozu genellikle kötüdür.
Iridocorneal endothelial (ICE) syndromes are a group of disorders typically affecting a single eye, predominantly seen in middle-aged women, and characterized by the abnormal proliferation of the corneal endothelial layer. This syndrome is examined under three subtypes: Chandler syndrome, progressive iris atrophy, and Cogan-Reese syndrome. Viral infections, particularly Herpes simplex and Epstein-Barr, are thought to play a triggering role in its pathogenesis. The disease leads to peripheral anterior synechia, iris changes, and secondary glaucoma as endothelial cells proliferate uncontrollably to form a membrane extending over the anterior chamber angle and the iris. While Chandler syndrome is characterized predominantly by a "beaten silver" appearance on the cornea and significant corneal edema, progressive iris atrophy presents with corectopia, stromal thinning, and stretch or melting holes. Cogan-Reese syndrome is distinguished by characteristic pigmented nodules on the anterior surface of the iris and the loss of normal iris architecture. Specular and confocal microscopy are of critical importance in diagnosis. Glaucoma prevalence varies between 46% and 82%, and it is resistant to medical therapy. Management involves trabeculectomy, glaucoma drainage devices, and endothelial keratoplasty techniques (DMEK/DSAEK) for corneal decompensation; however, the long-term prognosis remains generally poor.
