İnsandaki Demodex Akarı ile Göz ve Diğer Hastalıklar Arasındaki İlişki

Özet

İnsan yüzündeki kıl folikülleri ve yağ bezlerinde yaşayan kalıcı ektoparazitler olan Demodex folliculorum ve D. brevis akarlarının morfolojisi, yaşam döngüsü, teşhisi ve çeşitli hastalıklarla ilişkisi üzerine yapılmış bilimsel çalışmalar derlenmiştir. 3-4 haftalık yaşam döngüsüne sahip olan bu mikroskobik canlılar, standart yüzeyel deri biyopsisi veya kirpik epilasyonu yöntemleriyle teşhis edilmektedir. Kültür ortamında ex vivo olarak yaşatılamamaları immünopatogenezlerinin tam olarak anlaşılmasını zorlaştırmaktadır. Epidemiyolojik verilere göre dünya nüfusunun %80-90'ında bulunan bu akarlar; blefarit, kuru göz, akne ve rozasea gibi oküler ve dermatolojik rahatsızlıkların gelişiminde önemli roller üstlenmektedir. Demodex yoğunluğu ilerleyen yaşla ve cildin yağ oranıyla paralellik gösterirken, cinsiyetler arasında belirgin bir fark bulunmamaktadır. Özellikle kanser, diyabet, kronik böbrek yetmezliği ve Behçet hastalığı gibi bağışıklık sisteminin baskılandığı durumlarda, parazit popülasyonunda ve enfestasyon riskinde ciddi bir artış gözlenmektedir. Tedavisinde alkol ve antiseptiklere dirençli olan bu akarlara karşı en etkili ajan çay ağacı yağı ve onun aktif bileşeni olan Terpinen-4-ol'dür. Sonuç olarak, halk sağlığı açısından bu yaygın parazitin patogenezini, bulaş yollarını ve korunma yöntemlerini tam olarak aydınlatabilmek adına ileri düzey moleküler ve immünolojik kültür çalışmalarına ihtiyaç duyulmaktadır.

Scientific studies on the morphology, life cycle, diagnosis, and association with various diseases of Demodex folliculorum and D. brevis mites, which are permanent ectoparasites living in human hair follicles and sebaceous glands, have been reviewed. These microscopic organisms, which have a life cycle of 3-4 weeks, are diagnosed using standard superficial skin biopsy or eyelash epilation methods. The inability to keep them alive ex vivo in culture mediums makes it difficult to fully understand their immunopathogenesis. According to epidemiological data, these mites are found in approximately 80-90% of the world's population and play significant roles in the development of ocular and dermatological disorders such as blepharitis, dry eye, acne, and rosacea. While Demodex density runs parallel with advancing age and skin oiliness, no significant difference is observed between genders. Particularly in immunosuppressed conditions such as cancer, diabetes, chronic renal failure, and Behçet's disease, a serious increase in parasite population and infestation risk is detected. In its treatment, tea tree oil and its active component Terpinen-4-ol are the most effective agents against these mites, which are resistant to alcohol and antiseptics. Consequently, advanced molecular and immunological culture studies are required to fully elucidate the pathogenesis, transmission routes, and prevention methods of this common parasite in terms of public health.
