Üriner Sistem Enfeksiyonları
Özet
Üriner sistem enfeksiyonları (ÜSE), bakteriyel etkenlerin neden olduğu ve hekimlerin klinik pratikte en sık karşılaştığı enfeksiyonlar arasında yer almaktadır. Kadınlarda daha yaygın görülmekle birlikte, özellikle ileri yaşlardaki erkeklerde prostat hipertrofisine bağlı olarak sıklığı artış göstermektedir. Enfeksiyonun en yaygın bulaş mekanizması patojenlerin periüretral bölgeden yukarı doğru tırmandığı asendan yoldur. Vakaların yüzde 85'inden fazlasında temel etken Escherichia coli olarak tanımlanırken, hastane kaynaklı veya tekrarlayan komplikasyonlu enfeksiyonlarda Proteus, Pseudomonas ve Klebsiella gibi dirençli türler de izole edilmektedir. Klinik olarak asemptomatik bakteriüri, akut sistit ve ateş ile yan ağrısıyla seyreden akut piyelonefrit şeklinde sınıflandırılır. Tanı sürecinde idrarın mikroskobik incelenmesi, piyüri ile hematüri varlığı ve temiz orta akım idrarından yapılan idrar kültürü altın standarttır. Tedavi yönetimi ise hastanın yaşına, gebelik durumuna ve enfeksiyonun anatomik yerine göre empirik olarak TMP-SMX, siprofloksasin veya fosfomisin gibi antimikrobiyal ajanlarla planlanmaktadır. Zamanında ve doğru antibiyotik seçimi yapılmadığı takdirde hastalık, ürosepsis tablosuna ilerleyerek mortaliteye yol açabilmektedir.
Urinary tract infections (UTIs) are the most common bacterial infections encountered by physicians in clinical practice. Although more prevalent in females, their incidence increases significantly in elderly males due to benign prostatic hypertrophy. The primary pathogenesis involves the ascending pathway, where pathogens colonize the periurethral area and migrate upward into the urinary bladder. Escherichia coli is isolated as the leading causative agent in more than 85% of cases, whereas Proteus, Pseudomonas, and Klebsiella species are commonly identified in nosocomial or recurrent complicated infections. Clinically, UTIs are categorized into asymptomatic bacteriuria, acute cystitis, and acute pyelonephritis, which presents with fever and flank pain. The diagnosis relies heavily on microscopic examination of the urine to detect pyuria and hematuria, along with a clean-catch midstream urine culture. Management involves empirical antimicrobial therapy with agents like TMP-SMX, ciprofloxacin, or fosfomycin, tailored to the patient's age, pregnancy status, and the location of the infection. Inappropriate or delayed treatment can lead to urosepsis, posing a substantial risk of mortality.
