Nekrotik Daimi Matür Dişlerde Rejeneratif Endodontik Tedaviler

Özet

Rejeneratif endodontik tedaviler (RET), doku mühendisliği ilkelerini kullanarak hasarlı veya nekrotik pulpa dokusunun sağlıklı ve canlı vital dokularla yer değiştirmesini hedefleyen, günümüz diş hekimliğindeki en heyecan verici ve biyolojik tabanlı gelişmelerden biridir. Geleneksel kök kanal tedavileri yüksek başarı oranları sunsa da dişin canlılığını geri kazandıramamakta, nörovasküler sistemi yeniden oluşturamamakta ve madde kayıpları ile post-operatif kırık risklerini artırmaktadır. Geçmişte daha çok kök gelişimi tamamlanmamış immatür dişlerde uygulanan rejenerasyon prosedürleri, doku mühendisliğindeki ilerlemeler sayesinde artık nekrotik daimi matür (kök gelişimi tamamlanmış) dişlerde de başarıyla uygulanmaktadır. Matür dişlerdeki RET uygulamalarında, kök kanallarının dezenfeksiyonu mekanik preparasyon ve kimyasal irrigasyonla (özellikle EDTA kullanımıyla) immatür dişlere kıyasla daha etkili şekilde sağlanmakta; periapikal lezyonların iyileşmesi, semptomların giderilmesi ve duyarlılık testlerine olumlu yanıtlar alınması gibi yüksek başarı oranları elde edilmektedir. Histolojik çalışmalar, tedavi sonrasında kanal içerisinde pulpa benzeri doku yerine sement, periodontal ligament ve kemik benzeri vital dokuların oluştuğunu, bu dokuların da enfeksiyonlara karşı immünoinflamatuar savunma mekanizması ve duyusal innervasyon yeteneği taşıdığını göstermektedir. Sonuç olarak RET, matür dişleri yabancı maddelerle doldurmak yerine konağın doğal yara iyileşme sürecini teşvik eden, cerrahi olmayan geleneksel endodontik tedavilere alternatif, güvenli ve umut verici bir tedavi seçeneğidir.

Regenerative endodontic treatments (RET) represent one of the most exciting, biologically based developments in modern dentistry, aiming to replace damaged or necrotic pulp tissue with healthy and vital tissues by utilizing tissue engineering principles. Although conventional root canal treatments offer high success rates, they cannot restore pulp vitality, recreate the neurovascular system, and they increase the risks of tissue loss and post-operative fractures. While regenerative procedures were historically focused on immature teeth with incomplete root development, advancements in tissue engineering have enabled successful applications in necrotic permanent mature teeth with completed root development. In mature teeth, root canal disinfection is achieved more effectively through mechanical preparation and chemical irrigation (especially using EDTA) compared to immature teeth, yielding high success rates including periapical lesion healing, symptom resolution, and positive responses to vitality tests. Histological studies demonstrate that instead of pulp-like tissue, vital tissues such as cementum, periodontal ligament, and bone-like tissues form within the canal space, providing immunoinflammatory defense mechanisms against infections and sensory innervation. Consequently, rather than filling root canals with foreign materials, RET promotes the host's natural wound healing process, serving as a safe, promising, and viable alternative to conventional non-surgical endodontic treatments.
