Romatoid Artrit’in Histopatolojik Etkileri, Tanı ve Tedavi Yöntemleri

Özet

Romatoid artrit (RA), başta eklemler olmak üzere kalp, akciğer ve böbrek gibi eklem dışı organları da etkileyebilen kronik, sistemik ve otoimmün bir inflamatuvar hastalıktır. Dünya genelinde prevelansı %1-1,5 civarında olan hastalık, tipik olarak 30-50 yaş aralığında ve kadınlarda 2 ila 4 kat daha sık görülür. Genetik (HLA-DR4/HLA-DRB1) ve sigara gibi çevresel faktörlerin etkileşimiyle gelişen hastalıkta, proteinlerin sitrülinasyonu ve anti-sitrülinlenmiş protein antikorlarının (ACPA) oluşumu temel immünolojik süreçleri oluşturur. Histopatolojik olarak sinoviyal membranda hiperplazi, anjiyogenez, pannus oluşumu ve inflamatuvar hücre infiltrasyonu gözlenir; salınan matriks metalloproteinazlar (MMP) ve osteoklast aktivasyonu eklem kıkırdağı ile kemik yıkımına yol açar. Klinik olarak simetrik çoklu eklem ağrısı, şişlik ve sabah tutukluğu ile karakterize olan RA; kardiyovasküler, solunum ve nörolojik sistem komorbiditeleri nedeniyle mortalite oranını artırır. Tanı aşamasında klinik bulguların yanı sıra RF, anti-CCP, ESH ve CRP testleri ile radyolojik Sharp skoru değerlendirmelerinden yararlanılır. Kesin bir tedavisi bulunmayan RA’in yönetiminde, EULAR hedefe yönelik tedavi yaklaşımı doğrultusunda semptomları hafifletmek ve eklem hasarını onarmak amacıyla konvansiyonel sentetik DMARD'lar (metotreksat), kortikosteroidler, biyolojik ajanlar (TNF ve IL-6 inhibitörleri) ve JAK inhibitörleri bireysel olarak uygulanmaktadır.

Rheumatoid arthritis (RA) is a chronic, systemic, and autoimmune inflammatory disease that primarily targets the joints but can also damage extra-articular organs such as the heart, lungs, and kidneys. With a global prevalence of approximately 1-1.5%, it typically manifests in individuals aged 30-50 and occurs 2 to 4 times more frequently in women. Developed through the interplay of genetic predisposition (HLA-DR4/HLA-DRB1) and environmental factors like smoking, the disease features protein citrullination and the production of anti-citrullinated protein antibodies (ACPA) as core immunologic processes. Histopathologically, the synovial membrane exhibits hyperplasia, angiogenesis, pannus formation, and inflammatory cell infiltration, where released matrix metalloproteinases (MMPs) and osteoclast activation cause joint cartilage and bone destruction. Clinically characterized by symmetrical multi-joint pain, swelling, and morning stiffness, RA increases mortality rates due to cardiovascular, respiratory, and neurological comorbidities. Diagnosis relies on clinical symptoms alongside RF, anti-CCP, ESR, and CRP tests, combined with radiological Sharp score evaluations. Lacking a definitive cure, RA management utilizes personalized regimens including conventional synthetic DMARDs (methotrexate), corticosteroids, biological agents (TNF and IL-6 inhibitors), and JAK inhibitors under the EULAR treat-to-target strategy to alleviate symptoms and prevent joint damage progression.
