Konik Işınlı Tomografinin Endodontide Kullanım Alanı
Synopsis
Konik Işınlı Bilgisayarlı Tomografi (KIBT), iki boyutlu panoramik ve periapikal radyografilerin süperpozisyon ve geometrik distorsiyon gibi sınırlamalarını aşarak endodontide tanı ve tedavi planlamasında devrim yaratmıştır. X-ışını kaynağının tek bir dönüşüyle yüksek çözünürlüklü, üç boyutlu veri hacmi sağlayan bu teknoloji; geleneksel bilgisayarlı tomografiye kıyasla daha hızlı ve düşük maliyetlidir. Endodontik durumlarda, hastanın maruz kaldığı radyasyon dozunu ve doku hacmini en aza indirmek adına yalnızca küçük görüş alanına (FOV) sahip taramalar önerilir. KIBT; geleneksel yöntemlerin yetersiz kaldığı erken dönem apikal periodontitis tespiti, karmaşık kök kanal anatomilerinin ve ek kanalların belirlenmesi, dental travmalar ile horizontal kök kırıklarının teşhisi ve iç-dış kök rezorpsiyonlarının bukko-palatal yayılımının değerlendirilmesinde belirgin bir üstünlük ve yüksek hassasiyet gösterir. Ancak, metal restorasyonların neden olduğu artefaktlar görüntü kalitesini etkileyebilir. Sonuç olarak KIBT, klinik muayenenin yetersiz kaldığı durumlarda tanı doğruluğunu artırarak en uygun tedavi planının oluşturulmasını sağlar.
Cone-Beam Computed Tomography (CBCT) has revolutionized diagnosis and treatment planning in endodontics by overcoming the limitations of two-dimensional panoramic and periapical radiographs, such as superposition and geometric distortion. Providing high-resolution, three-dimensional data volumes with a single rotation of the X-ray source, this technology is faster and less costly compared to conventional computed tomography. In endodontic cases, only small field of view (FOV) scans are recommended to minimize patient radiation dose and tissue volume. CBCT demonstrates significant superiority and high sensitivity in detecting early-stage apical periodontitis where traditional methods fail, identifying complex root canal anatomy and additional canals, diagnosing dental trauma and horizontal root fractures, and evaluating the bucco-palatal extent of internal-external root resorptive lesions. However, artifacts caused by metallic restorations can affect image quality. Consequently, CBCT enhances diagnostic accuracy when clinical examination is insufficient, enabling the development of the most appropriate treatment plan.
