Özet

Genel anestezi, modern anestezi pratiğinin temel taşlarından biri olup hipnoz, analjezi, amnezi, kas relaksasyonu ve otonom stabilite gibi birbiriyle etkileşimli bileşenlerin dengeli biçimde sağlanmasını gerektirir. Bu bölümde, genel anestezinin klinik amaçları ve fizyolojik temelleri açıklandıktan sonra indüksiyon ajanları (propofol, barbitüratlar, benzodiyazepinler, ketamin, etomidat), opioidler, inhalasyon anestezikleri, total intravenöz anestezi (TIVA) ve nöromüsküler blokerler farmakolojik özellikleri ve klinik kullanım alanları bakımından ele alınmaktadır. Standart indüksiyon protokollerinin yanı sıra kardiyak riskli, septik ve hipovolemik hastalarda uygulanan özelleştirilmiş indüksiyon stratejilerine ve hızlı seri indüksiyon prensiplerine yer verilmiştir. İntraoperatif hemodinamik yönetim, sıvı tedavisi yaklaşımları, akciğer koruyucu ventilasyon stratejileri ve prone pozisyonun fizyolojik etkileri ayrı başlıklar altında değerlendirilmiştir. Anesteziden güvenli uyanma, rezidüel nöromüsküler blok yönetimi ve perioperatif kriz durumları (hipotansiyon, miyokard enfarktüsü, anafilaksi, farkındalık, malign hipertermi) sistematik biçimde özetlenmiştir. Bölüm, ERAS protokolleri, veri temelli hemodinamik izlem, BIS monitörizasyonu, yapay zekâ destekli klinik karar sistemleri ve kişiselleştirilmiş anestezi yaklaşımlarını tartışarak disiplinin gelecekteki yönelimlerine dair bütüncül bir bakış açısı sunmaktadır. Bu içerik, anesteziyoloji pratiğinde güncel kanıta dayalı klinik karar verme sürecine katkı sağlamayı amaçlamaktadır.
