Özet

Kedi ve köpek acil hekimliğinde klinik vaka yönetimi, yaşamı tehdit eden durumların erken tanınması, hızlı stabilizasyonun sağlanması ve altta yatan nedenin etkin şekilde tedavi edilmesine dayanmaktadır. Bu bölümde, acil klinik uygulamalarda karar verme sürecini standartlaştırmak amacıyla şok, solunum sistemi acilleri, kedilerde alt üriner sistem hastalığı (FLUTD) ve travma olgularına yönelik algoritmik yaklaşımlar sunulmuştur. Şok olgularında hipovolemik, kardiyojenik, obstrüktif ve dağılımsal şok tiplerinin patofizyolojisi, klinik bulguları, tanısal değerlendirme yöntemleri ve tedavi prensipleri ayrıntılı olarak ele alınmıştır. Solunum sistemi acillerinde respiratorik triyaj, oksijen desteği, görüntüleme yöntemleri ve hastalığa özgü tedavi seçenekleri sistematik bir algoritma çerçevesinde değerlendirilmiştir. FLUTD'li kedilerde klinik değerlendirme, laboratuvar analizleri, görüntüleme yöntemleri, obstrüksiyonun acil yönetimi, sıvı-elektrolit tedavisi, analjezi ve uzun dönem yönetim stratejileri açıklanmıştır. Travmalı hastalarda ise ABCDE yaklaşımı temel alınarak havayolu, solunum, dolaşım, nörolojik durum ve çevresel değerlendirme basamakları ile acil resüsitasyon ilkeleri özetlenmiştir. Bölüm sonunda sunulan FLUTD ve travma olguları, algoritmik yaklaşımın klinik uygulamadaki kullanımını örnekleyerek tanı ve tedavi sürecinin sistematik şekilde yürütülmesine katkı sağlamaktadır. Sonuç olarak algoritma temelli klinik vaka yönetimi, acil hastalarda erken karar verilmesini kolaylaştırmakta, tedavi başarısını artırmakta ve hasta prognozunun iyileştirilmesine önemli katkı sağlamaktadır.
