Kedi ve Köpeklerde Nörolojik Aciller
Özet
Nörolojik aciller, kedi ve köpeklerde yüksek morbidite ve mortalite ile seyreden, hızlı tanı, etkin triyaj ve zamanında tedavi gerektiren yaşamı tehdit eden klinik durumlardır. Bu bölümde veteriner acil hekimliğinde en sık karşılaşılan nörolojik aciller; nöbetler, status epileptikus, küme nöbetleri, travmatik beyin hasarı, akut paraliziler, omurilik yaralanmaları ve intrakraniyal basınç artışı güncel bilimsel veriler ışığında kapsamlı olarak ele alınmıştır. Nörolojik acillerin yönetiminde hava yolu, solunum ve dolaşımın (ABC yaklaşımı) hızla stabilize edilmesi, sistemik bozuklukların düzeltilmesi ve erken nörolojik değerlendirmenin prognoz açısından kritik öneme sahip olduğu vurgulanmıştır. Nöbetlerin etiyolojisi, tanısal yaklaşımı ve basamaklı antikonvülsan tedavi algoritmaları ayrıntılı olarak açıklanırken, status epileptikusun acil yönetiminde benzodiazepinler, ikinci ve üçüncü basamak antiepileptik tedaviler ile yoğun bakım uygulamalarına yer verilmiştir. Travmatik beyin hasarında primer ve sekonder beyin hasarının patofizyolojisi, serebral perfüzyonun korunması, intrakraniyal basınç yönetimi ve nörolojik monitörizasyon ilkeleri değerlendirilmiştir. Ayrıca akut nörolojik defisitlerin lokalizasyonu, nörolojik muayene basamakları, ileri görüntüleme yöntemleri, beyin omurilik sıvısı analizi ve yoğun bakım izlemi ayrıntılı olarak sunulmuştur. Erken tanı, uygun stabilizasyon, hedefe yönelik medikal tedavi ve yakın monitörizasyonun nörolojik iyileşmeyi artırdığı ve mortaliteyi azalttığı vurgulanmıştır. Bu bölüm, kedi ve köpeklerde nörolojik acillerin tanı ve tedavisinde, sistematik ve güncel bir klinik rehber sunmayı amaçlamaktadır
