Özet

Bu bölümde, kedi ve köpeklerde görülen renal ve ürolojik aciller güncel veteriner acil ve yoğun bakım uygulamaları doğrultusunda kapsamlı olarak ele alınmıştır. Akut böbrek hasarı (ABH) ile üretral obstrüksiyonun etiyolojisi, patogenezi, epidemiyolojisi, klinik bulguları, tanısal yaklaşımları ve tedavi prensipleri ayrıntılı biçimde değerlendirilmiştir. Akut böbrek hasarında erken tanının ve altta yatan nedenin belirlenmesinin prognoz üzerindeki belirleyici etkisi vurgulanırken, glomerüler filtrasyon hızının değerlendirilmesi, serum kreatinin ve sistatin C gibi biyobelirteçler ile NGAL, NAG ve GGT gibi tübüler hasar belirteçlerinin klinik önemi açıklanmıştır. Ayrıca IRIS akut böbrek hasarı evrelendirme sistemi doğrultusunda hastaların sınıflandırılması ve uygun tedavi stratejilerinin belirlenmesi ele alınmıştır. Tedavi bölümünde sıvı resüsitasyonu, elektrolit ve asit-baz bozukluklarının düzeltilmesi, hiperkaleminin acil yönetimi, renal replasman tedavileri ve leptospirozis ile etilen glikol toksikasyonu gibi spesifik etiyolojilere yönelik yaklaşımlar güncel kanıtlar ışığında sunulmuştur. Bölümün ikinci kısmında ise özellikle erkek kedilerde yaşamı tehdit eden en önemli üriner sistem acillerinden biri olan üretral obstrüksiyon ayrıntılı olarak incelenmiş; patofizyolojisi, klinik bulguları, stabilizasyon ilkeleri, analjezi, sıvı tedavisi, dekompresif sistosentez, üriner kateterizasyon ve yeniden obstrüksiyonun önlenmesine yönelik medikal yaklaşımlar açıklanmıştır. Erken tanı, hızlı stabilizasyon ve sistematik acil hasta yönetiminin mortalite ve komplikasyon oranlarını azaltmadaki kritik rolü vurgulanmıştır. Bu bölüm, kedi ve köpeklerde renal ve ürolojik acillerin tanı, tedavi ve yoğun bakım yönetimi konusunda kapsamlı bir klinik rehber sunmaktadır.
