Kedi ve Köpeklerde Gastrointestinal Aciller
Özet
Bu bölümde, kedi ve köpeklerde görülen gastrointestinal sistem acilleri güncel veteriner acil ve yoğun bakım uygulamaları doğrultusunda kapsamlı olarak ele alınmıştır. Gastrik dilatasyon-volvulus (GDV), gastrointestinal obstrüksiyonlar, peritonit, gastrointestinal kanamalar ve akut pankreatit başta olmak üzere yaşamı tehdit eden gastrointestinal hastalıkların etiyolojisi, patogenezi, klinik bulguları, tanısal yaklaşımları ve tedavi prensipleri ayrıntılı olarak değerlendirilmiştir. Hastaların yönetiminde erken tanının, hızlı stabilizasyonun ve sistematik klinik yaklaşımın prognoz üzerindeki belirleyici rolü vurgulanmıştır. Fizik muayene bulgularının laboratuvar analizleri, abdominal radyografi, ultrasonografi, endoskopi ve gerektiğinde ileri görüntüleme yöntemleri ile birlikte değerlendirilmesinin doğru tanıya ulaşmadaki önemi ortaya konulmuştur. Tedavi bölümünde sıvı resüsitasyonu, elektrolit ve asit-baz dengesinin düzeltilmesi, analjezi, antiemetik ve gastroprotektif tedavi, uygun antibiyotik kullanımı, erken enteral beslenme, yoğun bakım uygulamaları ile cerrahi girişim endikasyonları güncel kanıtlara dayalı olarak sunulmuştur. Ayrıca sistemik inflamatuvar yanıt sendromu, sepsis, septik şok ve çoklu organ disfonksiyonu gibi komplikasyonların erken tanınması ve yönetiminin mortalitenin azaltılmasındaki kritik önemi vurgulanmıştır. Bu bölüm, kedi ve köpeklerde gastrointestinal sistem acillerinin tanı, tedavi ve yoğun bakım yönetiminde veteriner hekimlere kapsamlı ve güncel bir klinik rehber sunmaktadır.
