Askariasise Bağlı Dev Mega Kolonlu Mental Retarde İleus Hastasında Anestezi Yönetimi
Özet
Olgumuz yaklaşık iki aydır devam eden karın ağrısı, bulantı-kusma, kabızlık şikayetiyle acil servise başvuran 42 yaşında 180 cm boyunda 60 kg ağırlığında erkek hasta. Akut batın ön tanısıyla acil operasyon planlanan hasta genel anestezi altında ameliyata alındı. Yaklaşık üç saat süren Ascaris lumbricoides’e bağlı ileus, dev megakolon gelişen ve kolonun dörtte üçü disseke edilen hastaya ileostomi açıldı. Hasta operasyon sonrası entübe ve yüksek doz inotrop desteğiyle reanimasyon yoğun bakım ünitesine alındı. Yoğun bakım takiplerinde trakeostomi açılan hasta uzun bir yatışın ardından kaybedildi. Bu olgunun sunumundaki amaç endemik olmayan bir bölgede ve erişkin bir hastada dramatik seyreden bir askariasis enfeksiyonuna rastlanılmış olması, gecikmiş hastane başvurusu sonucu kolonun dev boyutlara ulaşması, yaklaşık olarak dörtte üçünün disseke edilmesi ve maalesef postoperatif dönemde mortal seyretmesidir. Ayrıca ileus ön tanılı akut batın olgularının ayırıcı tanısında askariazisin dikkate alınmasının önemi de vurgulanmaktadır.
