Miks Pektus Deformitesi Cerrahisinde Multimodal Ağrı Kontrolü Stratejisi: Rejyonel Blok Temelli Deneyim
Özet
Göğüs duvarı deformiteleri, hem estetik hem de kardiyopulmoner etkileri nedeniyle cerrahi girişim gerektiren kompleks durumlar arasında yer alır. Bu olguda, miks pektus deformitesi nedeniyle ardışık iki operasyon geçiren adolesan bir hastada uygulanan farklı rejyonel blok teknikleri (SAP+TTP ve parasternal blok) postoperatif analjezi açısından sunulmuştur. İlk cerrahide uygulanan SAP+TTP kombinasyonu, geniş anterior torasik alanı kapsayan rekonstrüktif girişime uygun bir yaklaşımla tercih edilmiş ve etkili analjezi sağlamıştır. İkinci cerrahide ise anatomik alanın sınırlı olması ve teknik kısıtlar nedeniyle yalnızca parasternal blok uygulanabilmiş, postoperatif dönemde analjezi düzeyi daha sınırlı kalmıştır. Bu olgu, blok seçiminin cerrahi alan ve anatomik yapılara göre bireyselleştirilmesinin önemini ortaya koymaktadır.
