Tanıdan Anestezi Yönetimine; Görme Bozukluğu ile Başvuran Preeklamptik Gebede Posterior Reversibl Ensefalopati Sendromu
Özet
Posterior Reversibl Ensefalopati Sendromu (PRES), preeklampsi ve eklampsi gibi obstetrik hipertansif durumların nadir ancak ciddi bir komplikasyonu olup ani baş ağrısı, nöbet, görme bozukluğu ve bilinç değişikliği ile karakterizedir. Bu olguda, 34 haftalık preeklamptik bir gebede gelişen PRES tablosu sunulmaktadır. Hastada şiddetli preeklampsi bulguları ve eklampsi nöbeti gelişmiş; acil sezaryen ile doğum gerçekleştirilip uygun antihipertansif tedavi, magnezyum sülfat infüzyonu ve destekleyici bakım ile başarılı yönetim sağlanmıştır. Olgunun yönetimi ve seyri, PRES patofizyolojisi, insidansı, tedavi yaklaşımları ile anestezi yönetimine dair güncel literatür bilgileri eşliğinde tartışılmaktadır.
