Tip II Mukopolisakkaridozlu Çocukta Bronkoskopi ve Zor Entübasyonun Yönetimi
Özet
Mukopolisakkaridozlar (MPS), glukozaminoglikan (GAG) birikimi nedeniyle ilerleyici seyir gösteren, solunum, kardiyovasküler, iskelet ve nörolojik sistemler de dahil olmak üzere anatomik anormalliklere ve organ disfonksiyonuna neden olan ve anestezik komplikasyon riskini artırabilen nadir görülen genetik hastalık grubudur. Bu hastaların anestezi yönetimi karmaşıktır. Özellikle havayolu yönetimi, ameliyattan önce fiberoptik havayolu incelemesini içerecek şekilde planlanmalıdır. Bu olgu sunumunda yabancı cisim aspirasyonu nedeniyle bronkoskopi planlanan ve zor entübasyon nedeniyle rijid bronkoskopi gerçekleştirilemeyen MPS Tip 2 tanılı çocuk hastada, video laringoskop ile entübasyon sonrası, pediatrik hastalara uygun fiberoptik bronkoskopumuz olmaması nedeniyle, fleksible fiberoptik sistoskop ile tüp içerisinden girilerek, büyüklüğü nedeniyle tüple beraber çıkarılmak zorunda kalınan, sonrasında tekrar video laringoskop ile entübe edilen vakayı sunduk.
