Memenin Filloides Tümörü

Özet

Filloides tümörleri, nadir görülen ve biyolojik davranışı değişkenlik gösterebilen fibroepitelyal meme tümörleridir. Genellikle hızlı büyüyen, ağrısız, iyi sınırlı kitleler şeklinde ortaya çıkar ve fibroadenomlarla karışabilir. Kesin tanı histolojik inceleme ile konur; bu nedenle şüpheli lezyonlarda kor biyopsi önerilir. Standart tedavi geniş cerrahi eksizyondur. Benign tümörlerde dar cerrahi sınır kabul edilebilirken, malign tümörlerde ≥1 cm negatif sınır tercih edilir. Aksiller lenf nodu tutulumu nadir olduğundan rutin aksiller diseksiyon önerilmez. Sınırda (borderline) ve malign tümörlerde adjuvan radyoterapi lokal nüksü azaltabilir. Kemoterapi yalnızca yüksek riskli veya metastatik hastalarda düşünülmeli ve yumuşak doku sarkomlarına yönelik protokoller uygulanmalıdır. Metastaz en sık akciğerlere yayılır, ancak seçilmiş hastalarda metastazektomi düşünülebilir. Beş yıllık sağkalım malign olgularda %60-80 arasında değişmektedir. Düzenli klinik takip, özellikle ilk iki yıl boyunca nüks açısından kritik öneme sahiptir.
