Prostat Kanserinin Patolojik Değerlendirilmesi
Özet
Prostat adenokarsinomu, erkeklerde en sık görülen solid malignite olup prostat kanserlerinin %95’inden fazlasını oluşturmaktadır. Klinik spektrum asemptomatik olgulardan metastatik hastalığa kadar uzanır. Histopatolojik inceleme tanı, prognoz ve tedavi planlamasında temel yöntemdir. Asiner adenokarsinom en yaygın alt tip iken; duktal, müsinöz, pleomorfik dev hücreli, sarkomatoid ve PIN-benzeri varyantlar nadir görülmekte, ancak genellikle daha agresif seyretmektedir. Tanıda histomorfolojik bulguların yanı sıra PSA, NKX3-1 ve AMACR gibi immünohistokimyasal belirteçler kullanılmaktadır. Gleason derecelendirme sistemi, prostat kanserinin prognostik sınıflandırmasında altın standarttır. 2005, 2014 ve 2019 güncellemeleri ile ISUP ve GUPS kriterleri geliştirilmiş; özellikle Gleason desen 4 oranı ve invaziv kribriform paternin varlığı, klinik açıdan yüksek prognostik değer taşımaktadır. Hastalık ilerlediğinde komşu organlara direkt invazyon, lenfatik yayılım ve özellikle kemik metastazı sık görülür. Moleküler patolojide DNA onarım genlerindeki bozukluklar, hedefe yönelik tedavilerin geliştirilmesine olanak sağlamış; PARP inhibitörleri ve immün kontrol noktası blokajı ileri evre olgular için umut vadetmiştir. Ayrıca, yapay zeka tabanlı algoritmaların patolojik değerlendirmeye entegrasyonu, gözlemciler arası farklılıkları azaltarak daha objektif bir derecelendirme potansiyeli sunmaktadır.
