İntrakranyal Hemorajilerde Tedavi Yaklaşımı
Özet
Spontan intrakranyal hemorajiler, serebral vasküler hastalığın en tehlikeli komplikasyonudur. Sıklıkları; ciddi yaralanmalara, uzun süreli sakatlıklara ve ölüme yol açabilmeleri nedeniyle hem tıbbi hem de sosyal açıdan çok büyük bir önem kazanmışlardır. Kesin tanıları, yerleri, geniş yayılımları ve eşlik eden bulguları çok uzun bir süre belirsizliğini korumaktaydı. Küçük ve derin yerleşimli hematomlarda bu faktörleri belirlemek imkansızdı. Ancak klinik tablo, risk faktörlerinin değerlendirilmesi ve görüntüleme yöntemlerindeki ilerleme ile günümüzde teşhis ve tedavi korkulacak bir durum olmaktan çıkmıştır. Hızlı tanı ve uygun tedavi hem mortaliteyi hem de morbiditeyi önemli ölçüde azaltmaktadır. Başvuran her hastanın öncelikle acil stabiliğzasyonu (havayolu, solunum ve dolaşım güvenliği) sağlanır, tanı koyulabilmesi için görüntüleme yöntemlerinden mümkün olduğunca hızlı yardım alınır; büyük hematomlarda ve intrakranyal basınç artışında genellikle cerrahi yöntem tercih edilirken, kan basıncı yönetimi, antikoagülanların etkisinin tersine çevrilmesi, nöbet profilaksisi ve kafa içi basıncın kontrol altına alınması gibi durumlarda medikal tedavi bize yardımcı olmaktadır.İntrakranyal basınç izlemi ve komplikasyonalrın yönetimi ise uygun yoğun bakım takibi ile sağlanabilmektedir.
