Metanol Zehirlenmesi ve Yönetimi

Özet

Metanol zehirlenmesi mortalite ve morbidite ile sonuçlanan global bir sağlık sorunudur. Kaçak olarak üretilmiş alkol tüketimi sonucu metanol zehirlenmesi tanısı alan olgu da fomepizole ulaşılamadığı için alternatif olarak etanol uygulanmış, beraberinde klinik olarak belirgin yüksek anyon açıklı metabolik asizodu olan hastaya hemodiyaliz de uygulanarak tedavisi başarılı ve hızlı bir şekilde uygulanmıştır. Metanol zehirlenmesi yönetiminde esas nokta, klinik belirti ve bulguların iyi tanınması ve beraberinde kuvvetli bir şüpheye sahip olarak gerekli tetkiklerin hızlandırılarak, tanının erken konmasıdır. Bu aşamada metanolün metabolitlerine metabolize olmasını engelleyecek tedavilerinde hızla başlanmış olması gerekmektedir. Aralıklı veya duruma göre sürekli hemodiyaliz uygulaması da durumu kritik olan veya kötüleşmesi öngöürlen hastalarda etkili bir tedavi yöntemidir.
