Sepsiste Yoğun Bakım Yönetimi
Özet
Sepsis; dolaşımda enfeksiyöz ajan olsun veya olmasın konağın bozulmuş immün yanıtını ile oluşan ve multiorgan yetmezliğe kadar ilerleyenleyen klinik bir durumdur. Sepsis ve bu duruma bağlı gelişen septik şok; yoğun bakım ünitelerinde mortalitesi oldukça yüksek seyretmektedir. Patofizyolojisinde çeşitli mekanizmalar olan sepsisin tanı ve tedavisi ise günümüze kadar birçok değişikliğe uğramıştır. Sepsisi erken tanımak ve erken hedefe yönelik tedaviye başlamak oldukça önemlidir. Tedaviye yanıtın dinamik olarak takip edilmesi ve elde edilen sonuçlara göre tedavinin gözden geçirilmesi gerekmektedir. Bu bölümde acil servise başvuran ve sepsis tanısı koyulup yoğun bakım takibi yapılan bir olgu eşliğinde sepsisin tanı ve tedavisi sunulacaktır.
Sepsis is a clinical condition that occurs with the host's impaired immune response, whether or not there is an infectious agent in the circulation, and progresses to multiorgan failure. Sepsis and septic shock that develops due to this condition; mortality remains quite high in intensive care units. The diagnosis and treatment of sepsis, which has various mechanisms in its pathophysiology, has undergone many changes until today. It is very important to recognize sepsis early and start targeted treatment early. The response to treatment should be followed dynamically and the treatment should be reviewed according to the results obtained. In this section, the diagnosis and treatment of sepsis will be presented with a case who was admitted to the emergency department and diagnosed with sepsis and followed up in intensive care.
